摘要
目的分析住院择期手术患者并发症发生情况及影响因素,为医院提升医疗质量精细化管理提供参考依据。方法收集某三甲医院2019年1月1日-2021年12月31日住院择期手术患者信息,分析手术并发症发生情况,运用χ^(2)检验进行单因素分析,选取有统计学意义的指标进行二分类Logistic回归分析。结果98027例择期手术病例中,发生手术并发症104例,并发症发生率0.11%;深静脉血栓29例、肺部感染28例和呼吸衰竭20例,例数较多。患者年龄、入院途径、手术级别、是否危重、是否转科、住院天数等与手术并发症明显相关(P<0.05);二分类Logistic回归分析显示,患者年龄≥65岁、四级手术、危重患者、住院天数长等是手术并发症发生的危险因素。结论某院择期手术并发症中,患者病情是否危重、手术级别和住院天数对手术并发症发生风险影响较大。医院应保障手术服务质量安全标准的应用,关注老年危重症患者,加强手术分级授权管理,控制超长住院日,持续提升医疗质量安全管理水平。
Objectives This study aims to analyze the incidence and influencing factors of complications in inpatients undergoing elective surgery to provide a reference for the refined management of the hospital.Methods The information on elective surgery patients in a tertiary hospital from January 1,2019 to December 31,2021 was collected and the occurrence of surgical complications was analyzed.The Chi-squared test was used for univariate analysis,and the indicators with statistical significance were selected for binary logistic regression analysis.Results Among the 98,027 cases of elective surgery,104 cases had surgical complications,and the complication rate was 0.11%.There were 29 cases of deep vein thrombosis,28 cases of pulmonary infection,and 20 cases of respiratory failure The number of cases was relatively large.The age of the patient,the route of admission,the grade of surgery,whether it was critical,whether it was transferred to another department,and the length of hospitalization were significantly related to surgical complications(P<0.05).Dichotomous Logistic regression analysis showed that patients aged 65 years old,grade 4 surgery,critically ill patients,and long hospital stays were risk factors for surgical complications.Conclusions In elective surgery in a certain hospital,the risk of surgical complications is greatly affected by whether it is critical,the level of surgery,and the length of hospital stay.Hospitals should ensure the application of quality and safety standards for surgical services,pay attention to elderly critically ill patients,strengthen the management of surgical classification and authorization,control excessively long hospital stays,and continuously improve the level of medical quality and safety management.
