肺部超声在极低出生体重儿无创通气监测中的应用

摘要 目的探究肺部超声在极低出生体重儿无创通气监测中的应用价值。方法选择2021年4月—2022年4月九江市妇幼保健院新生儿科极低出生体重儿需行无创通气治疗的60例作为研究对象,所有患儿均采用肺部超声及胸部X线检查。分析极低出生体重儿肺部超声评分,不同X线分级下极低出生体重儿与肺部超声的相关性,肺部超声在极低出生体重儿使用无创通气监测中的预测价值。结果行肺部超声检查可见极低出生体重儿双肺评分为(28.69±7.58)分,左肺评分为(14.68±2.83)分,右肺评分为(14.21±3.10)分,双侧肺评分为(13.97±3.20)分;当X线分级越高,肺部超声评分越高,差异有统计学意义(P<0.05),Spearman秩相关性分析显示,二者呈正相关(r=0.859,P<0.05);R OC曲线结果表明,肺部超声在极低出生体重儿无创通气监测中具有较高的预测效能,当肺部超声评分为12.75分时,预测敏感度为87.16%,特异度为63.79%,AUC值为0.870。结论肺部超声在极低出生体重儿无创通气监测应用中预测价值较高,其预测结果与X线分级存在一定相关性,可为临床诊疗提供可靠的参考依据。 Objective To explore the application value of pulmonary ultrasound in monitoring noninvasive ventilation of very low birth weight infants.Methods From April 2021 to April 2022,60 cases of very low birth weight infants who needed non-invasive ventilation were enrolled in the neonatology department of Jiujiang Maternal and Child Health Hospital.All the infants were examined by lung ultrasound and chest X-ray.To analyze the lung ultrasound score of very low birth weight infants,to analyze the correlation between very low birth weight infants and lung ultrasound at different X-ray grades,and to analyze the predictive value of lung ultrasound in the monitoring of non-invasive ventilation of very low birth weight infants.Results The lung ultrasound examination showed that the lung score of very low birth weight infants was(28.69±7.58),the left lung score was(14.68±2.83),the right lung score was(14.21±3.10),and the bilateral lung score was(13.97±3.20).The higher the X-ray grade was,the higher the lung ultrasound score was,and the difference was statistically significant(P<0.05).Spearman rank correlation analysis showed that there was a positive correlation between the two(r=0.859,P<0.05).ROC curve results showed that pulmonary ultrasound had high predictive efficacy in the monitoring of non-invasive ventilation of very low birth weight infants.When the pulmonary ultrasound score was 12.75,the predictive sensitivity,specificity and AUC value were 87.16%,63.79%and 0.870,respectively.Conclusion Pulmonary ultrasound has a high predictive value in monitoring noninvasive ventilation of very low birth weight infants,and it can be used in monitoring noninvasive ventilation of very low birth weight infants.The prediction results are correlated with X-ray grading,which can provide a reliable reference for clinical diagnosis and treatment.
