摘要
目的:探讨程序化疼痛护理模式在口腔癌放疗患者中的应用。方法:将2020年1月—2021年12月在福建医科大学肿瘤临床医学院治疗的60例口腔癌放疗患者按随机数表法分为两组,各30例,对照组采用常规护理,观察组采用程序化疼痛护理模式。对比两组的疼痛及睡眠情况、口腔黏膜炎发生情况、心理状态、癌症疲乏程度、生存质量,均在1个放疗周期后(护理后)进行评价。结果:观察组护理后数字疼痛评分法(numerical rating scale,NRS)评分、匹茨堡睡眠质量指数量表(Pittsburgh sleep quality index,PSQI)评分均明显低于对照组,差异有统计学意义(P<0.05)。观察组护理后口腔黏膜炎总发生率低于对照组,差异有统计学意义(P<0.05)。观察组护理后焦虑自评量表(self-rating anxiety scale,SAS)评分、抑郁自评量表(self-rating depression scale,SDS)评分均明显低于对照组,差异有统计学意义(P<0.05)。观察组护理后情感疲乏评分、认知疲乏评分、躯体疲乏评分明显低于对照组,差异有统计学意义(P<0.05)。护理后,观察组功能领域评分、整体生活质量评分明显高于对照组,症状领域评分、单一症状因子评分明显低于对照组,差异有统计学意义(P<0.05)。结论:程序化疼痛护理模式在口腔癌放疗患者中的应用效果显著,能有效缓解疼痛程度,降低口腔黏膜炎发生率及严重程度,减轻机体疲乏程度,改善生存质量。
Objective:To explore the application of programmed pain nursing model in patients with oral cancer undergoing radiotherapy.Method:From January 2020 to December 2021 in Clinical Oncology School of Fujian Medical University for the treatment of 60 patients with oral cancer radiotherapy press numeric table method were randomly divided into two groups,30 cases in each group.The control group was treated with routine nursing,and the observation group was treated with programmed pain nursing.Pain and sleep,oral mucositis,psychological condition,cancer fatigue degree and quality of life were compared between the two groups,all were evaluated after 1 radiotherapy cycle(after nursing).Result:After nursing,the scores of numerical rating scale(NRS)and Pittsburgh sleep quality index(PSQI)in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The total incidence of oral mucositis in the observation group after nursing was lower than that in the control group,the difference was statistically significant(P<0.05).The self-rating anxiety scale(SAS)score and self-rating depression scale(SDS)score in the observation group after nursing were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After nursing,the scores of emotional fatigue,cognitive fatigue,and physical fatigue in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After nursing,the functional area score and overall quality of life score in the observation group were significantly higher than those in the control group,while the symptom area score and single symptom factor score were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The application effect of programmed pain nursing model in patients with oral cancer undergoing radiotherapy is significant,which can effectively relieve th
