摘要
目的探讨早期康复训练在口腔癌游离腓骨瓣移植术后患者下肢的功能恢复中的应用效果。方法选取2019年7月—2020年8月医院收住的口腔癌游离腓骨瓣行上、下颌骨修复重建患者40例,将其按基本资料组间具有可比性的原则分为观察组和对照组,各20例。对照组给予常规康复护理,观察组在对照组基础上给予早期康复训练,分别于手术前、术后1周、术后2周、术后3个月进行客观测量和运用Enneking下肢功能量表行进行主观评价,比较两组供侧区域功能恢复情况。结果观察组患者在术后恢复过程中,踝关节内翻、外翻、背伸关节活动度和拇趾伸展肌力方面的恢复速度快于对照组,差异有统计学意义(P<0.05)。两组患者Enneking下肢功能量表评分显示,经康复训练介入的观察组患者相关评分随时间变化而升高,恢复速度及质量均优于对照组,差异有统计学意义(P<0.05)。结论游离腓骨瓣术后患者实施早期康复训练,可有效促进患者下肢功能的恢复。
Objective To explore the application effect of early rehabilitation training on the functional recovery of lower limbs in patients with oral cancer after free fibular flap repair and reconstruction of the upper and lower jaws.Methods 40 patients with oral cancer who underwent upper and lower jaw repair and reconstruction with free fibular flap admitted to the hospital from July 2019 to August 2020 were selected.They were divided into an observation group and a control group according to the principle of comparability between basic data groups,with 20 patients in each group.The control group was given routine rehabilitation care,and the observation group was given early rehabilitation training on the basis of the control group.Measurement and subjective evaluation were carried out before the operation,1 week after the operation,2 weeks after the operation,and 3 months after the operation using the Enneking lower limb function scale.The condition in donor side function zone recovery were compared between the two groups.Results During the postoperative recovery process,the recovery speed of ankle joint varus,valgus,back extension joint range of motion,and toe extension muscle strength in the observation group was faster than the control group,the difference was statistically significant(P<0.05).The score of the Enneking Lower Limb Function Scale for two groups of patients showed that the observation group with rehabilitation training intervention showed a significant increase in related scores over time,and the recovery speed and quality were better than the control group,with a statistically significant difference.Conclusion Early rehabilitation training can effectively promote the recovery of lower limb function in patients after free fibular flap surgery.
