摘要
目的分析结缔组织病(CTD)合并间质性肺病(ILD)患者病情活动度与外周血纤维化指标、T淋巴细胞亚群水平的相关性。方法回顾性分析本院2020年3月—2022年3月收治的142例CTD-ILD患者,根据患者病情活动度分组,其中临床缓解期患者69例为对照组,活动期患者73例为观察组,比较2组患者外周血纤维化指标[透明质酸(HA)、Ⅳ型胶原(Ⅳ-C)、层黏连蛋白(LN)和Ⅲ型前胶原肽(PⅢP)]和T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平,并分析其与患者病情活动度的相关性。结果观察组患者外周血HA和IV-C水平高于对照组患者(P<0.05);观察组患者外周血CD8^(+)水平高于对照组患者(P<0.05),观察组患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平低于对照组患者(P<0.05);患者病情活动度与HA(r=0.633,P<0.05)、Ⅳ-C(r=0.535,P<0.05)、CD8^(+)(r=0.303,P<0.05)成正相关,与CD3^(+)(r=-0.176,P<0.05)、CD4^(+)(r=-0.197,P<0.05)、CD4^(+)/CD8^(+)(r=-0.498,P<0.05)成负相关。结论外周血纤维化指标(HA、Ⅳ-C)及T淋巴细胞亚群水平(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))与CTD-ILD患者病情活动度关系密切,其可作为反映患者病情活动情况的重要指标。
Objective This paper aims to analyze the correlation between disease activity and peripheral blood fibrosis indexes,T lymphocyte subsets in patients with connective tissue disease(CTD)and interstitial lung disease(ILD).Methods A retrospective analysis was performed on the 142 patients with CTD-ILD admitted to the hospital between March 2020 and March 2022.According to disease activity,they were divided into control group(69 cases,clinical remission stage)and observation group(73 cases,active stage).The levels of peripheral blood fibrosis indexes[hyaluronic acid(HA),typeⅣcollagen(Ⅳ-C),laminin(LN),precollagenⅢpeptide(PⅢP)]and T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))between the two groups were compared.And their correlation with disease activity was analyzed.Results The levels of peripheral blood HA andⅣ-C in observation group were higher than those in control group(P<0.05).The level of peripheral blood CD8^(+)in observation group was higher than that in control group(P<0.05),while the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were lower than those in control group(P<0.05).The disease activity was positively correlated with HA(r=0.633,P<0.05),Ⅳ-C(r=0.535,P<0.05)and CD8^(+)(r=0.303,P<0.05),while negatively correlated with CD3^(+)(r=-0.176,P<0.05),CD4^(+)(r=-0.197,P<0.05)and CD4^(+)/CD8^(+)(r=-0.498,P<0.05).Conclusion Peripheral blood fibrosis indexes(HA,Ⅳ-C)and T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))are closely related to disease activity in patients with CTD-ILD,which can be applied as important reference indexes of disease activity.
