摘要
目的探讨超声定量评估胰腺回声增强与血脂异常的相关性。方法选取北部战区总医院自2020年5月至2021年4月收治的312例行腹部超声检查患者为研究对象。根据是否发生血脂异常将患者分为A组(n=252)与B组(n=60)。A组患者未发生血脂异常,B组患者发生血脂异常。记录并比较两组患者血生化指标及超声指标,包括总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、胰腺体部平均亮度(MPBB)、肝周脂肪平均亮度(MPHFB)、胰腺-肝周脂肪指数(PPHFI)、丙氨酸氨基转移酶(ALT)、天冬氨酸转氨酶(AST)等。采用Pearson相关性分析法检验PPHFI与血生化指标的相关性。采用偏相关分析法补充分析PPHFI与血生化指标的相关性。结果B组患者MPBB、PPHFI、ALT、AST、TC、TG及LDL水平均高于A组,差异均有统计学意义(P<0.05)。Pearson相关性分析显示,PPHFI与ALT、AST、TC、TG、LDL水平均呈正相关性(r=0.230、0.223、0.441、0.537、0.379,P<0.05)。控制年龄、性别等因素后,通过偏相关性分析法发现,PPHFI仍与ALT、AST、TC、TG、LDL水平呈正相关性(r=0.233、0.247、0.363、0.552、0.320,P<0.05)。结论PPHFI升高与血脂异常显著相关,超声诊断PPHFI升高对于血脂异常具有重要临床价值,可为临床预防提供依据。
Objective To investigate the correlation between enhanced echo of pancreas and dyslipidemia.Methods 312 patients who underwent abdominal ultrasonography from May 2020 to April 2021 in General Hospital of Northern Theater Command were selected as subjects.Patients were divided into group A(n=252)and group B(n=60)according to whether dyslipidemia occurred.There was no dyslipidemia in group A,and dyslipidemia in group B.Blood biochemical indexes and ultrasonic indexes were recorded and compared between the two groups.They included total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL),low density lipoprotein(LDL),mean pancreatic body brightness(MPBB),mean perihepatic fat brightness(MPHFB),pancreato-perihepatic fat index(PPHFI),alanine amino transferase(ALT),aspartate transaminase(AST),etc.Pearson correlation analysis was used to test the correlation between PPHFI and blood biochemical indexes.Partial correlation analysis was used to supplement the correlation between PPHFI and blood biochemical indexes.Results The levels of MPBB,PPHFI,ALT,AST,TC,TG and LDL in group B were higher than those in group A,with statistical significance(P<0.05).Pearson correlation analysis showed that PPHFI was positively correlated with ALT,AST,TC,TG and LDL levels(r=0.230,0.223,0.441,0.537,0.379,P<0.05).After controlling age and gender,PPHFI was still positively correlated with ALT,AST,TC,TG and LDL(r=0.233,0.247,0.363,0.552,0.320,P<0.05)by partial correlation analysis.Conclusion Elevated PPHFI is significantly correlated with dyslipidemia.Ultrasound diagnosis of elevated PPHFI has important clinical value for dyslipidemia and can provide evidence for clinical prevention.
