摘要 目的:探讨超声诊断弥漫性胰腺回声增强的临床意义,分析胰腺回声与血脂、血糖等生化指标关系。方法:选取2019年1月—2021年12月在成武县人民医院进行健康体检的7292例受检者,超声诊断为弥漫性胰腺回声增强患者共256例为病例组,并随机选取同期胰腺回声正常者1020名作为对照组,整理受检者超声及实验室检查资料,分析两组患者间高血脂、高血糖发生率的差异;进一步探讨胰腺回声强度与血脂、血糖、体重指数等指标相关性。结果:病例组与对照组间性别及高密度脂蛋白(high density lipoprotein,HDL)水平差异均不显著(P>0.05)。病例组空腹高血糖、高胆固醇、高甘油三酯的发生率均高于对照组(22.5%vs7.9%、12.3%vs5.9%、55.2%vs17.3%),差异具有统计学意义(P<0.05);病例组TC、TG、LDL、BMI及空腹血糖水平均较对照组显著升高(P<0.05)。胰腺回声强度与TC、TG及LDL、BMI等呈正相关关系(P<0.05)。控制性别后,偏相关显示胰腺回声强度仍与TG、LDL、BMI及血糖水平呈显著正相关关系(P<0.05),回声强度与TG相关性最强(r=0.597,P<0.01)。结论:超声诊断弥漫性胰腺回声增强对于提示无症状性高血脂、高血糖有重要的临床价值,可为临床预防提供重要依据。 Objective To explore the clinical significance of ultrasound diagnosis of diffuse pancreatic echo enhancement,and to analyze the relationship between pancreatic echo and biochemical indicators such as blood lipids and blood sugar.Methods 7292 subjects who underwent physical examinations in Chengwu County People's Hospital from January 2019 to December 2021 were selected.A total of 256 patients with diffuse pancreatic echogenicity diagnosed by ultrasound were selected as the case group.The normal pancreatic echo was randomly selected during the same period.A total of 1020 subjects were selected as the control group.The ultrasound and laboratory examination data of the subjects were sorted out,and the differences in the incidence of hyperlipidemia and hyperglycemia between the two groups were analyzed;the correlation between pancreatic echo intensity and indicators such as blood lipids,blood glucose,and body mass index was further explore.Results There were no significant differences in gender and high density lipoprotein(HDL)levels between the case group and the control group(P>0.05).The incidence rates of fasting hyperglycemia,hypercholesterolemia and hyper-triglyceride in the case group were higher than those in the control group(22.5%vs 7.9%,12.3%vs 5.9%,55.2%vs 17.3%)and the differences were statistically significant(P<0.05);TC,TG,LDL,BMI and fasting blood glucose levels in the case group were significantly higher than those in the control group(P<0.05).Pancreatic echo intensity was positively correlated with TC,TG,LDL and BMI(P<0.05).After controlling for gender,the partial correlation showed that the pancreatic echo intensity was still positively correlated with TG,LDL,BMI and blood glucose levels(P<0.05),and the echo intensity had the strongest correlation with TG(r=0.597,P<0.01).Conclusion Ultrasound diagnosis of diffuse pancreatic echogenicity has important clinical value in prompting asymptomatic hyperlipidemia and hyperglycemia,and can provide an important basis for clinical prevention.
