摘要
目的研究拟编排和验证食道胃曲张静脉破裂出血救治的高保真临床模拟场景。方法研究分为三个阶段:根据住培医师规范化培训教学大纲要求构建模拟场景,以帮助学习者增加食道胃曲张静脉破裂出血诊断和救治的知识,包括识别症状/体征、抢救和后续治疗。由6名评委进行内容验证,以及由上海交通大学医学院附属新华医院的3名住培医师进行场景测试。通过内容效度指数对数据进行分析,评委满意度>0.90认为是满意的。结果观察到该模拟场景是合适的,评估结果显示该模拟场景的条理性、严谨性、完整性和规范性得分分别为92分、93分、94分和92分。并根据评委和参与测试学员的建议,在文本条理性、严谨性、完整性和规范性方面对场景和测试进行了一些调整。结论被验证的模拟场景有望成为临床情景模拟教学的促进工具。
Objective To establish and test a high-fidelity clinical simulation scenario for the treatment of esophageal and gastric variceal vein rupture and bleeding.Methods The methodological study was divided into three stages:simulation scenarios were constructed according to the requirements,including recognition of symptoms/signs,rescue and follow-up treatment.Content verification was conducted by 6 judges,and scene test was conducted by 3 training resident of Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine.The data was analyzed by content validity index,and the results showed that the judges were satisfied if their satisfaction was greater than 0.90.Results The simulation scenario was observed to be suitable,and the overall value of logicality,stringency,integrity,and normativity scores was 92,93,94 and 92,respectively.According to the suggestions of the judges,some adjustments were made to the scenes and tests in terms of logicality,stringency,integrity,and normativity.Conclusion The verified simulation scenario is expected to be a tool to promote clinical simulation teaching.
