摘要
目的:探讨清热止痒方治疗湿热蕴结证特应性皮炎(AD)患儿的临床效果。方法:研究类型为前瞻性对照研究,将湖南中医药大学附属第二医院皮肤科2021年6月-2022年5月收治的86例湿热蕴结证AD患儿随机分为对照组与研究组,每组43例。对照组采用丙酸氟替卡松乳膏外涂治疗,研究组采用清热止痒方治疗,疗程共4周。对比两组临床效果,入组时及治疗后的特应性皮炎评分(scoring of atopic dermatitis,SCORAD)、中文版儿童皮肤病生活质量指数(CDLQI)、细胞因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-2(IL-2)、白细胞介素-17(IL-17)]及血清总IgE水平。结果:计算脱落与剔除后,对照组共39例,研究组共40例纳入最终分析。研究组临床疗效优于对照组,差异有统计学意义(P<0.05);研究组治疗后SCORAD的皮损严重程度、皮肤病变范围、瘙痒与睡眠丧失平均程度得分及总分均低于对照组,差异均有统计学意义(P<0.05);研究组治疗后CDLQI中的治疗、睡眠、人际关系、学习和假期、休闲娱乐、症状和感受评分均低于对照组,差异均有统计学意义(P<0.05);研究组治疗后的TNF-α、IL-17及总IgE水平均低于对照组,差异均有统计学意义(P<0.05)。结论:与丙酸氟替卡松乳膏比较,清热止痒方治疗湿热蕴结证AD患儿的效果更好,可减轻临床症状,提高生活质量,下调TNF-α、IL-17及总IgE水平。
Objective:To explore the clinical efficacy of Qingre Zhiyang Formula in the treatment of children with atopic dermatitis(AD)of damp-heat accumulation syndrome.Method:The research type was a prospective controlled study.86 AD of damp-heat accumulation syndrome children received in the Dermatology of Second Affiliated Hospital of Hunan University of Chinese Medicine from June 2021 to May 2022 were randomly divided into the control group and the research group,with 43 cases in each group.The control group were treated with external application of Fluticasone Propionate Cream;the research group were treated with Qingre Zhiyang Formula.The course of treatment was 4 weeks.The clinical efficacy,scoring of atopic dermatitis(SCORAD),Chinese children's dermatology life quality index(CDLQI),cytokines[tumor necrosis factor-α(TNF-α),interleukin-2(IL-2),interleukin-17(IL-17)]and serum total IgE levels at enrollment and after treatment in the two groups were compared.Result:After calculating dropout and exclusion,a total of 39 cases in the control group and 40 cases in the research group were included in the final analysis.The curative effect of the research group was better than that of the control group,the difference was statistically significant(P<0.05).The scores of skin lesion severity,skin lesion range,pruritus and sleep loss and total score of SCORAD after treatment were lower than those of control group,the differences were statistically significant(P<0.05).The scores of treatment,sleep,interpersonal relationship,learning and vacation,leisure and recreation,symptoms and feelings of CDLQI in the research group were lower than those in the control group after treatment,the differences were statistically significant(P<0.05).The levels of TNF-α,IL-17 and total IgE in the research group after treatment were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Compared with Fluticasone Propionate Cream,Qingre Zhiyang Formula has better efficacy in the treatment of AD of d
