摘要
目的:分析三维超声影像技术对阴道分娩后早期女性盆底特征及耻骨直肠肌损伤的评估价值。方法:选取2022年1—12月平原县第一人民医院收治的80例初次妊娠并经阴道分娩后3~5 d产妇为观察组,另选我院同期80名无生育史的育龄女性为常规组,均予以三维超声检查,观察两组女性的盆底特征,并根据有无耻骨直肠肌损伤对观察组进行分组,对比其各项参数。结果:三维超声影像结果显示,观察组盆膈裂孔前后径(anteroposterior diameter,AP)、盆膈裂孔左右径(left to right diameter,LR)、盆膈裂孔面积(hiatal area,HA)均显著大于常规组,耻骨直肠肌厚度(puborectal muscle thickness,T-PR)显著小于常规组(P<0.01)。观察组中耻骨直肠肌损伤组AP、LR、HA均显著大于未损伤组,T-PR显著小于未损伤组(P<0.01)。结论:三维超声影像技术对盆底支持系统结构能够起到良好评估效果,有助于临床及早发现阴道分娩后女性的耻骨直肠肌损伤等异常状况,为疾病改善提供重要依据。
Objective To analyze the value of three-dimensional ultrasound imaging technology in the evaluation of female pelvic floor characteristics and puborectal muscle injury in the early stage after vaginal delivery.Methods 80 women of childbearing age who were admitted to the First People's Hospital of Pingyuan County from January to December 2022 for the first time of pregnancy and 3 to 5 days after vaginal delivery were selected as the observation group,and 80 women of childbearing age who had no reproductive history in the same period of our hospital were selected as the routine group.All of them were examined by three-dimensional ultrasound to observe the pelvic floor characteristics of the two groups of women,and the observation group was divided according to whether there was puborectal muscle injury,and its parameters were compared.Results Three-dimensional ultrasound imaging results showed that the anteroposterior diameter of the pelvic septal hiatus(AP),left to right diameter of the pelvic septal hiatus(LR),area of the pelvic septal hiatus(HA)in the observation group were significantly greater than those in the conventional group,and the puborectal muscle thickness(T-PR)was significantly smaller than that in the conventional group(P<0.01).In the observation group,the AP,LR,HA of the puborectal muscle injury group were significantly higher than those of the non-injured group,and the T-PR was significantly smaller than that in the non-injured group(P<0.01).Conclusion Three-dimensional ultrasound imaging technology can play a good role in evaluating the structure of pelvic floor support system,which is helpful to detect the abnormal conditions such as puborectal muscle injury in women after vaginal delivery as early as possible,and provide an important basis for disease improvement.
