摘要
目的:经会阴超声观察产后盆底结构变化,评估不同分娩方式对产后早期盆底结构和功能的影响。方法:选取在徐州医科大学附属医院分娩并于产后6周在本院复查的初产妇共60例,根据分娩方式不同分为阴道分娩组和选择性剖宫产组。同时选取30例健康体检的未育女性作为对照组。经会阴二维及三维超声对各组盆底结构参数进行比较分析。结果:阴道分娩组膀胱颈下移距离(bladder neck down distance,BND)、宫颈下移距离(cervix down distance,CD)明显大于选择性剖宫产组,P<0.05;阴道分娩组盆膈裂孔(pelvic diaphragm hiatus,PH)各参数均大于选择性剖宫产组,P<0.05。结论:经会阴二维联合三维超声能全面评估产后早期盆底结构的变化,是观察女性盆底结构有效的影像学方法。
Objective:To observe the changes in postpartum pelvic floor structure with transperineal ultrasound,to assess the impact of different delivery methods on early postpartum pelvic floor structure and function.Methods:60 primiparae were recruited as a case group at their follow-up visit at 6 weeks after delivery in the Affiliated Hospital of Xuzhou Medical University.The subjects included 30 cases of vaginal delivery and 30 cases of elective cesarean section.A contemporary cohort of 30 healthy nulliparas in the physical examination was selected as the control group.The data of all groups were compared and analyzed.Results:For the comparison of BND and CD,the measurement of the vaginal delivery group was larger than that of the elective cesarean section,P<0.05;the parameters of PH in the vaginal delivery group were larger than those in the elective cesarean section group P<0.05.Conclusion:Transperineal two dimensional and three dimensional ultrasound is an effective evaluation imaging method of female pelvic support system,with the ability of assessing the changes of early postpartum pelvic floor structure and function comprehensively.
