针刀干预对膝骨关节炎兔关节腔内压和滑膜血管增生的影响 被引量:2

摘要 目的:探讨针刀对于膝骨关节炎(KOA)兔关节腔内压和滑膜血管增生的作用。方法:新西兰雄兔随机分为正常组、模型组、针刀组和电针组,每组各7只。左后肢膝关节制动6周制备退行性膝骨关节炎模型。正常组、模型组不予干预,针刀组、电针组各干预3周。测量兔关节腔内压,用HE染色法观察兔滑膜形态学,并用Western Blot技术观测兔滑膜中HIF-1α、VEGF蛋白含量。结果:与正常组相比,模型组关节腔内压显著升高,差异具有统计学意义(P<0.01),滑膜HE染色出现血管增生改变,滑膜HIF-1α、VEGF蛋白含量显著升高,差异具有统计学意义(P<0.01);干预结果表明,与模型组相比,针刀组、电针组关节腔内压显著降低,差异具有统计学意义(P<0.01),滑膜HE染色血管增生减轻,滑膜HIF-1α、VEGF蛋白含量显著降低,差异具有统计学意义(P<0.01或P<0.05);与电针组相比,针刀组关节腔内压显著降低,差异具有统计学意义(P<0.01),滑膜HE染色血管增生减少明显,滑膜VEGF蛋白含量明显降低,差异具有统计学意义(P<0.05),HIF-1α蛋白含量有下降趋势,差异无统计学意义(P>0.05)。结论:针刀通过降低关节腔内压,进而减少滑膜血管增生,发挥对KOA的治疗作用。 Objective:To investigate the effect of acupotomy on intra-articular pressure and synovial angiogenesis in rabbits with knee osteoarthritis(KOA).Methods:New Zealand male rabbits were randomly divided into the normal group,the model group,the acupotomy group and the electro-acupuncture(EA)group,with 7 rabbits in each group.The degenerative KOA model was established by immobilizing the left hind knee for 6 weeks.The normal group and the model group were not intervened,and the acupotomy group and the EA group were intervened for 3 weeks.The intra-articular pressure was measured,the morphology of synovium was observed by HE staining,and the protein contents of HIF-1αand VEGF in synovium were detected by Western Blot technique.Results:Compared with those in the normal group,the intra-articular pressure was significantly increased(P<0.01),the synovial HE staining showed changes in angiogenesis,and the synovial protein contents of HIF-1αand VEGF were significantly increased in the model group(P<0.01).Compared with those in the model group,the intra-articular pressure was significantly decreased(P<0.01),the synovial angiogenesis was alleviated,and the protein contents of HIF-1αand VEGF in the synovium were significantly decreased in the acupotomy group and the EA group(P<0.01,P<0.05).Compared with those in the EA group,the intra-articular pressure was significantly reduced(P<0.01),the synovial angiogenesis was significantly reduced,and the synovial protein of VEGF was significantly reduced in the acupotomy group(P<0.05);there was a downward trend in HIF-1αprotein in the acupotomy group,but in which there was no statistical difference between the EA group and the acupotomy group(P>0.05).Conclusion:Acupotomy can reduce the intra-articular pressure,thereby reducing the synovial angiogenesis,and play a therapeutic role in treatment of KOA.
