摘要
[目的]分析男性艾滋病病毒感染者和病人(HIV/AIDS)中合并梅毒感染的危险因素,为防控HIV/AIDS合并梅毒感染提供参考。[方法]以横断面研究方法对昆明市2013-2021年同时报告了HIV/AIDS和梅毒的以及仅报告了HIV/AIDS的男性病例进行研究。采用Excel 2007和SPSS 19.0软件整理和分析数据,采用单因素和多因素Logistic回归分析合并感染的危险因素。[结果]8636例男性HIV/AIDS确诊病例纳入研究,其中HIV/AIDS合并梅毒感染的698例(占8.1%),未合并梅毒感染的7938例(占91.9%)。多因素Logistic回归分析显示,相比于已婚、工人等职业、异性传播,未婚(aOR=1.81)、羁押人员(aOR=4.27),同性传播(aOR=12.67)、双性传播(aOR=2.55)、性+吸毒传播(aOR=2.38)是男性HIV/AIDS合并梅毒感染的危险因素(P均<0.05)。通过性病门诊(aOR=2.29)发现的男性合并感染病例显著多于其他途径(P<0.05)。[结论]未婚、同性传播、双性传播、性+吸毒传播是昆明市男性HIV/AIDS合并梅毒感染的危险因素,应开展综合干预措施防控HIV/AIDS中梅毒感染和传播。
Objective To analyze the risk factors of men infected with HIV and HIV-infected male patients with syphilis infection and provide reference for the prevention and control of HIV/syphilis co-infection.Methods The cross-sectional study was conducted to analyze men infected with HIV and HIV-infected men with syphilis infection that were reported from 2013 to 2021 in Kunming. Excel 2007 and SPSS19.0 software were used to collate and analyze data. Univariate and multivariate logistic regression analysis were used to analyze risk factors for syphilis infection in HIV/AIDS-infected male patients.Results 8636 male HIV/AIDS confirmed cases were included in the research. Among them, 698 cases(8.1%)were infected with HIV/AIDS and syphilis and 7938 cases(91.9%)were only infected with HIV without syphilis infection. According to multivariate logistic regression, risk factors of syphilis infection in HIV/AIDS-infected male patients were being unmarried(aOR=1.81),detainees(aOR=4.27),homosexual transmission(aOR=12.67),bisexual transmission(aOR=2.55),sexual and drug abuse transmission(aOR=2.38),(P<0.05),comparing with those who were married, workers and doing other occupations and heterosexual transmission. The male co-infection cases reported by sexually transmitted diseases(STDs)out-patient clinics(aOR=2.29)were significantly more than those who were infected by other ways(P<0.05).Conclusions Being unmarried, homosexual transmission, bisexual transmission, sexual and drug abuse transmission are risk factors of syphilis infection in HIV/AIDS-infected male patients in Kunming. Comprehensive intervention measures should be adopted to prevent and control infection and transmission of syphilis in male HIV/AIDS patients.
