关于国际高血压领域系统评价近20年研究热点的可视化分析

Visual analysis of the recent 20 years of research hotspots in the international systematic review of hypertension

摘要 目的 分析近20余年国际高血压相关系统评价/Meta分析文献的研究趋势和研究热点以及宏观的知识结构。方法 以Web of Sicence(WOS)数据库作为数据源,对1999年1月至2019年12月所收录的高血压的相关系统评价文献进行检索统计分析,并进行关键词社会网络图谱及聚类分析和突显词分析。结果 共检出相关文献1278篇,近20年每年发文量呈平稳上涨趋势。通过对关键词进行聚类分析,结果显示当年的热点主要集中以下6大类:干预措施(常用抗高血压药化学药物、替代疗法、手术治疗、非药物干预)、传统危险因素(家族史、肥胖、饮食、环境)、新危险因素(血脂有关成分、内皮素、代谢相关因子、情绪)、基因相关性(基因多态性、血管紧张素原、基因变异)、合并症(左室肥厚、高尿酸血症、阿尔茨海默病)、特殊人群高血压(妊娠性高血压、隐性高血压、子痫、老年收缩期高血压、高血压肾损害、高血压糖尿病、难治性高血压等)、社区管理等领域。通过对突显词分析,发现突显程度最强的依次是“危险因素”、“偏倚”和“汉族”。文献共被引分析发现被引用率最高的主要是方法学文章。结论 通过对WOS数据库的基础分类分析描述和共现图谱网络分析,直观展示了近20年国际高血压的热点主要集中在高血压的危险因素、基因与血压相关性,靶器官的损害、肾动脉交感神经消融术等研究方面。此外,非药物干预和中医药替代疗法也愈来愈受到关注。口服降压药的随机对照试验和高血压的新危险因素研究在未来仍具有很大的研究空间。 Objective To understand the research trends and hot spots of international systematic review/Meta-analysis of hypertension and the macro knowledge structure in recent 20 years. Methods The systematic review literature on hypertension collected from January 1999 to the end of 2019 was analyzed using the WOS database, the social network graph of keywords, clustering analysis, and prominent words analysis were carried out.Results A total of 1278 relevant articles were identified. The annual volume of texts in the recent 20 years showed a plateau and increasing trend. By clustering the keywords, the results showed that the hot spots in the current year mainly focused on the following six main categories: interventions(commonly used antihypertensive drugs,chemical drugs, replacement therapy, surgical treatment, non-pharmacological intervention), traditional risk factors(family history, obesity, diet, environment), new risk factors(lipid-related components, endothelin, metabolismrelated factors, mood) Areas such as genetic associations(gene polymorphisms, angiotensinogen, gene variants),comorbidities(left ventricular hypertrophy, hyperuricemia, Alzheimer’s disease), hypertension in special populations(gestational hypertension, cryptic hypertension, eclampsia, geriatric systolic hypertension, hypertensive renal impairment,hypertensive diabetes, refractory hypertension, etc.), and community management. Through the analysis of the salience of the word, it was found that the most substantial order of salience was "risk factor", "bias", "and "Han Chinese".Literature co citation analysis, the most highly cited primary methodological articles were found. Conclusion Through the description of fundamental classification analysis and co-occurrence map network analysis of the WOS database, it can be seen that the hot spots of international hypertension in the past 20 years mainly focus on the risk factors of hypertension, the correlation between genes and blood pressure, target organ damage, renal artery sympathetic nervous sys
