血清抗缪勒管激素和硫酸脱氢表雄酮在辅助诊断PCOS临床价值中的应用 被引量:3

Application of serum anti-mullerian hormone and dehydroepiandrosterone sulfate in the clinical value of auxiliary diagnosis of PCOS

摘要 目的探讨血清抗缪勒管激素(AMH)和硫酸脱氢表雄酮(DHEA-S)在辅助诊断PCOS临床价值中的应用。方法选择2021年1月至2022年5月于阜阳市人民医院生殖医学科收治的124例PCOS患者纳入PCOS组,另选择62例健康育龄女性纳入对照组,记录两组患者BMI、年龄等一般资料,重点检测血清AMH、DHEA-S等内分泌激素相关指标。采用Pearson相关分析AMH、DHEA-S与各因素的相关性,Logistic多元回归分析影响育龄女性发生PCOS的危险因素。结果PCOS组患者BMI、初潮年龄、月经周期紊乱者比例、TC、TG、LDL-C、AMH、LH、LH/FSH、T、DHEA-S、E2显著高于对照组,FSH显著低于对照组;AMH<10ng/mL组FSH水平显著高于AMH≥10ng/mL组,而LH、LH/FSH、T水平显著低于AMH≥10ng/mL组,且两组AMH检测值相比差异显著;DHEA-S<233μg/dL组T水平显著低于DHEA-S≥233μg/dL组,E2水平显著高于DHEA-S≥233μg/dL组,且两组DHEA-S检测值相比同样差异显著;Pearson相关分析结果显示,AMH表达水平与FSH呈负相关性,与T呈正相关性,DHEA-S表达水平与T呈正相关性,而与E2呈负相关性;Logistic多元回归分析表明AMH、DHEA-S是影响育龄女性发生PCOS的独立危险因素;ROC曲线表明血清AMH+DHEA-S联合检测可显著提高检测的PCOS的灵敏度和特异度。结论PCOS在临床表现上具有多样性和高度异质性,AMH、DHEA-S参与了PCOS的发生发展,导致患者在卵泡发育和排卵方面产生障碍,可通过检测其表达水平变化提高对PCOS的诊断价值。 Objective To investigate the clinical value of serum anti-mullerian hormone(AMH)and dehydroepiandrosterone sulfate(DHEA-S)in auxiliary diagnosis of PCOS.Methods A total of 124 patients with PCOS who were admitted to the department of reproductive medicine,Fuyang People’s Hospital from January 2021 to May 2022 were selected and included in the PCOS group,another 62 healthy women of childbearing age were selected to be included in the control group.The general data such as BMI and age of the patients in the two groups were recorded,and the serum AMH,DHEA-S and other endocrine hormone-related indicators were mainly detected.Pearson correlation was used to analyze the correlation between AMH,DHEA-S and various factors,and Logistic multiple regression analysis was used to analyze the risk factors of PCOS in women of childbearing age.Results The BMI,age at menarche,proportion of menstrual cycle disorder,TC,TG,LDL-C,AMH,LH,LH/FSH,T,DHEA-S and E2in the PCOS group were significantly higher than those in the control group,and FSH was significantly lower than that in the control group.The level of FSH in the AMH<10 ng/mL group was significantly higher than that in the AMH≥10 ng/mL group,while the levels of LH,LH/FSH,and T were significantly lower than those in the AMH≥10 ng/mL group,and there were significant differences in AMH detection values between the two groups.The T level in the DHEA-S<233μg/dL group was significantly lower than that in the DHEA-S≥233μg/dL group,and the E2level was significantly higher than that in the DHEA-S≥233μg/dL group,and the DHEA-S detection values of the two groups were significantly different.The results of Pearson correlation analysis showed that the expression level of AMH was negatively correlated with FSH and positively correlated with T,and the expression level of DHEA-S was positively correlated with T,but negatively correlated with E2.Logistic multiple regression analysis showed that AMH and DHEA-S were independent risk factors for PCOS in women of childbearing age.Conclu
