摘要
目的 观察舒适心理护理联合多形式的健康教育模式对慢性呼吸衰竭(CRF)患者护理质量及预后的影响。方法 将162例CRF患者随机分为对照组和观察组,每组81例。对照组予常规护理干预,观察组予舒适心理护理联合多形式的健康教育模式。采用自我护理能力量表(ESCA)、圣乔治呼吸问卷(SGRQ)、焦虑自评量表(SAS)、抑郁自评量表(SDS)、自制护理质量问卷对两组患者干预前、出院时与随访3个月的自我护理能力、生活质量、心理状态及护理质量等指标进行比较;检测并比较两组患者干预前、出院时与随访3个月的1 s用力呼气容积(FEV_(1))、用力肺活量(FVC)、1 s用力呼气容积所占用力肺活量比例(FEV_(1)/FVC)、动血氧饱和度(SaO_(2))、动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))水平等指标。结果 与干预前比较,出院时、随访3个月后两组FEV_(1)、FVC、FEV_(1)/FVC、SaO_(2)、PaO_(2)、自我护理能力各项评分及总分、生活质量总分、护理质量评分均升高(P<0.01),PaCO_(2)、SGRQ、SAS与SDS评分均下降(P<0.01),且以观察组更显著(P<0.01或0.05)。结论 舒适心理护理联合多形式的健康教育模式应用于CRF患者可辅助改善患者肺功能,提高其自我管理能力、生活质量及护理质量,缓解焦虑、抑郁情绪。
Objective To investigate the effect of comfortable psychological nursing combined with multi-form health education mode on nursing quality and prognosis of patients with chronic respiratory failure(CRF). Methods A total of 162CRF patients were randomly divided into the Control Group and the Observation Group, 81 cases in each group. The Control Group was given routine nursing while the Observation Group was given comfort psychological nursing combined with multiform health education mode. The self-care ability, quality of life, mental state and nursing quality of the two groups before intervention, at discharge and at 3 months after follow-up were compared by adopting such indicators as the Evaluation of Selfcare Ability Scale(ESCA), St. George’s Respiratory Questionnaire(SGRQ), Self-Rating Anxiety Scale(SAS), Self-Rating Depression Scale(SDS) and self-made nursing quality questionnaire. The FEV_(1), FVC, FEV_(1)/FVC, SaO_(2), PaCO_(2), PaO_(2)and other indicators of the two groups before intervention, at discharge and at 3 months after follow-up were detected and compared.Results The FEV_(1), FVC, FEV_(1)/FVC, SaO_(2), PaO_(2), the scores of self-care abilities and total ESCA score, total score of quality of life and the score of nursing quality of both groups were increased at discharge and 3 months after follow-up compared with those before intervention(P<0.01), and the PaCO_(2), SGRQ, SAS and SDS scores were decreased at discharge and 3 months after follow-up compared with those before intervention(P<0.01), and the Observation Group showed a more significant change(P<0.01 or 0.05). Conclusion The application of comfort psychological nursing combined with multi-form health education model in CRF patients can help improve the pulmonary function of patients, improve their self-management ability and quality of life as well as nursing quality, and relieve anxiety and depression.
