摘要
目的总结沈敏鹤主任中医师诊治消化系统肿瘤用药规律。方法纳入2018年11月1日至2021年10月31日就诊于浙江中医药大学附属第一医院沈敏鹤主任中医师门诊的消化系统肿瘤患者283例,共3919诊次,收录患者基本信息,建立临床方剂数据库,对用药频次、药物性味及药物类别进行描述性分析,同时对用药频次≥137的高频药物进行系统聚类分析。结果283例患者3919诊次共用药338味,累计用药频次29618次,其中党参用药频数最多,其次为甘草、茯苓、半夏、白术、柴胡等;药味以甘味药最多;药性以平性药使用次数最多;药物类别使用频数最多者为补虚药,其次为利水渗湿药、清热药等;系统聚类分析将高频药物分为22类,涉及方剂六君子汤、小柴胡汤等,涉及药对天麻与川芎、木香与砂仁等。结论沈敏鹤中医师主任治疗消化系统肿瘤善用健脾疏肝之法,佐以利水渗湿、清热解毒、解表化痰等法,临证用药常以甘、平配伍。
Objective To summarize the rule of prescriptions of chief Chinese medicine practitioner Shen Minhe in treating tumors of digestive system.Methods Collected 283 cases of digestive system tumor treated by the First Affiliated Hospital of Zhejiang Chinese Medical University,Shen Minhe from November 12018 to October 312021,a total of 3919 clinical visits.Basic information of the patients was collected,the database of clinical prescriptions was established to conduct descriptive analysis on the cumulative frequency,natures and tastes,and categories of medicinals.Meanwhile,the high-frequency drugs whose frequency≥137 were analyzed by cluster analysis.Results Totally 338 medicinals were prescribed in 3919 clinical visits with 283 patients.The cumulative frequency of medicinals was 29618,in which Radix Codonopsis had the highest frequency,followed by Liquorice,Poria Cocos,Pinellia Ternata,Atractylodes Macrocephala,Bupleurum Root and others.The sweet taste medicine and the flat nature medicine were the most used.The most frequently used medicinal category was tonify deficiency medicine,followed by the categories of clearing damp and promoting clearing the heat,and other diuresis.The medicines were classified into 22 categories by cluster analysis,involved formulas including Liujunzi Decoction,Xiaochaihu Decoction and others.Medicinal pairs included Gastrodia elata and Chuanxiong,radix aucklandiae and amomum villosum lour and others.Conclusion Director Shen Minhe is good at invigorating the spleen and soothing the liver in treating digestive system tumor and associates with methods of eliminating dampness and diuresis,clearing heat and removing toxicity,relieving superficies dissipating phlegm and others.The clinical medication is usually combined sweet taste medicine and the flat nature medicine.
