摘要
“咽源性咳嗽”首次由耳鼻喉专家干祖望教授提出,临床以咽喉不适为主要表现,可闻及患儿频繁清嗓之声,可见咽后壁淋巴滤泡增生,其主要特点为喉头一痒即咳,不痒不咳,且病程多较长,短则数周,长则数月,严重影响患儿的身心健康,家长亦因多处求医但疗效欠佳而焦虑。西医多采用抗感染、抗过敏及局部雾化治疗,疗效欠佳,而中医则通过四诊合参、辨证施治,疗效显著。近年来,小儿咽源性咳嗽的患病率逐渐升高。为提高对小儿咽源性咳嗽的认识,更好诊治咽源性咳嗽患儿,对近10年各专家学者对小儿咽源性咳嗽的辨证治疗经验进行探析。多数中医学者认为咽源性咳嗽多归因于“肺失宣降,肺气上逆”,其发病与肺、脾、肝、肾脏器密切相关,肺气失宣、脾失运化、肝失疏泄、肺肾阴虚均可引起患儿咽源性咳嗽。临床医家多从肺脾、肺肝、肺肾论治,将其辨证为风邪犯肺证、痰热互结证、阴虚火旺证。吾师根据多年临床经验,认为咽源性咳嗽初起以风寒风热袭咽为主,进而以痰热蕴结肺脾为主,后期以肺阴不足、脾胃虚弱为主,治以疏风宣肺、止咳利咽、清热化痰、滋阴润肺、健脾养咽;提出咽源性咳嗽患儿需注重日常防护,应做到戴口罩、避风寒、忌酸甜凉等。
“Pharyngeal cough”is first proposed by otolaryngology expert Professor GAN Zuwang.And clinically it is mainly manifested by throat discomfort,and the frequent throat clearance could be heard,and the lymphatic follicular hyperplasia of the posterior pharyngeal wall could be seen.Its main characteristics are that the throat is itchy and coughs occur when it is itching,and the course of the disease is longer,ranging from several weeks to several months.Therefore,it seriously affects the physical and mental health of the children,and the parents are also anxious because of poor medical treatment.Western medicine mostly uses antiinfection,anti-allergic and local atomization treatments,but the effect is not good,while traditional Chinese medicine uses four-diagnosis and treatment based on syndrome differentiation,and the effect is significant.In recent years,the prevalence of pediatric pharyngeal cough has gradually increased.In order to improve the understanding of pediatric pharyngeal cough,and to better diagnose and treat pharyngeal cough in children,the research conducted by various experts and scholars on pediatric cough of pharyngeal in the past 10 years has been explored.Most TCM practitioners believe that pharyngeal cough is mostly attributed to“Lung Qi(气)fails to descent,which leads to lung Qi reverse”,and its incidence is closely related to the lungs,spleen,liver,and kidney organs,and lung Qi deficiency,spleen dysfunction,liver failing in drainage,and lung and kidney Yin(阴)deficiency can cause pharyngeal cough in children.Clinicians mostly treat the disease from the lungs and spleen,lungs and liver,lungs and kidneys,and the disease is divided into the syndrome of wind evil attacking lung,the syndrome of interlocking phlegm-heat,and the syndrome of Yin deficiency and fire flourishing.According to many years of clinical experience,the master believes that pharyngeal cough is mainly caused by wind,cold wind and heat attacking the pharynx at the beginning,and then phlegm and heat accumulating in the l
