摘要
目的:探究高压氧联合醒脑静对急性一氧化碳(CO)中毒昏迷患者血浆高迁移率族蛋白B1(HMGB1)表达及迟发性脑病发生率的影响。方法:选择中国人民解放军联勤保障部队第九二五医院2020年4月至2021年4月收治的急性CO中毒昏迷患者30例作为研究对象。按照随机数表法将其分为A、B、C三组,每组各10例。A组予以高压氧治疗,B组予以醒脑静治疗,C组予以高压氧联合醒脑静治疗。比较三组治疗后血浆HMGB1的表达水平,3个月后通过回访及复诊评估三组迟发性脑病的发生情况。结果:治疗后1 d、3 d、5 d、7 d及14 d,C组血浆HMGB1的表达水平均显著低于A组、B组,差异有统计学意义(P<0.05)。3个月后通过回访及复诊评估得知,C组迟发性脑病的发生率为10.00%,显著低于A组的50%、B组的60%,差异有统计学意义(P<0.05)。结论:用高压氧联合醒脑静对急性CO中毒昏迷患者进行治疗能显著降低其血浆HMGB1的表达水平和迟发性脑病的发生率。
Objective:To investigate the effects of hyperbaric oxygen combined with Xingnaojing on the expression of high mobility group protein B1(HMGB1)in plasma and the incidence of delayed encephalopathy in coma patients with acute carbon monoxide(CO)poisoning.Methods:Thirty comatose patients with acute CO poisoning admitted to 925th Hospital of Joint Logistic Support Force of Chinese People's Liberation Army from April 2020 to April 2021 were selected as the research objects.According to the method of random number table,the patients were divided into three groups:A,B and C,with 10 cases in each group.Group A was given hyperbaric oxygen therapy,group B was given Xingnaojing therapy,and group C was given hyperbaric oxygen combined Xingnaojing therapy.Plasma HMGB1 expression levels were compared among the three groups after treatment,and the occurrence of delayed encephalopathy was evaluated by follow-up visit and return visit 3 months later.Results:On day 1,3,5,7 and 14 after treatment,the plasma HMGB1 expression level of group C was significantly lower than that of group A and B,with statistical significance(P<0.05).After 3 months,the incidence of delayed encephalopathy in group C was 10.00%,which was significantly lower than 50%in group A and 60%in group B,and the difference was statistically significant(P<0.05).Conclusion:Hyperbaric oxygen combined with Xingnaojing can significantly reduce the expression level of plasma HMGB1 and the incidence of delayed encephalopathy in coma patients with acute CO poison ing.
