摘要
目的观察百令胶囊联合肾衰宁颗粒在慢性肾功能衰竭(Chronic Renal Failure,CRF)中的临床应用价值。方法选取2019年1月—2020年12月期间南通大学第四附属医院盐城市第一人民医院肾内科收治的CRF患者106例,采用随机数字表法分为对照组和观察组,每组各53例。两组患者均给予血液透析(HD)治疗,对照组加用肾衰宁颗粒,观察组在对照组基础上加用百令胶囊。治疗16周后,观察比较治疗前后微炎症指标[超敏C反应蛋白(hypersensitive C-reactive Protein,hs-CRP)、白介素-6(Interleukin-6,IL-6)、白介素-8(Interleukin-8,IL-8)、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)]、营养指标[前白蛋白(Prealbumin,PA)、白蛋白(Albumin,ALB)、转铁蛋白(Transferrin,TRF)、血红蛋白(Hemoglobin,Hb)]、T淋巴细胞亚群指标[CD3^(+)、CD4^(+)、CD8^(+)和CD4^(+)/CD8^(+)]、肾纤维化指标[转化生长因子β1(Transforming Growth Factor-β,TFG-β1)、IV型胶原(Collage TypeⅣ,Col-IV)、纤维黏连蛋白(Fibronectin,FN)和结缔组织生长因子(Connective Tissue Growth Factor,CTGF)]和肾功能指标[尿素氮(Blood Urea Nitrogen,BUN)、血肌酐(Serum Creatinine,Scr)、24 h尿蛋白定量(24-Hour Urinary Protein Quantity,24 h UPQ)和肾小球滤过率(Estimated Glomerular Filtration Rate,eGFR)]变化情况及评估临床疗效。结果治疗后两组患者微炎症指标水平均较治疗前降低,差异有统计学意义(P<0.05);且观察组微炎症指标较对照组明显降低,差异有统计学意义(P<0.05)。治疗后对照组营养指标无明显改变(P>0.05),观察组营养指标水平提高(P<0.05);观察组营养指标较对照组明显提高,差异有统计学意义(P<0.05)。治疗后两组患者CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)指标均较治疗前升高,CD8^(+)指标较治疗前降低,差异有统计学意义(P<0.05);且观察组T淋巴细胞亚群指标较对照组改善更明显,差异有统计学意义(P<0.05)。治疗后两组患者肾纤维化指标水平均较治�
Objective To observe and analyze the value of Bailing Capsulescombined with Shenshuaining Granules in the treatment of chronic renal failure(CRF)and explore the mechanism of action.Methods The 106 CRF patients who were treated in the Department of Nephrology,Yancheng First People′s Hospital,The Fourth Affiliated Hospital of Nantong University were classified into the observation group(n=53)and control group(n=53)according to a randomnumber table.Both groups were treated with hemodialysis(HD).The control group was added with Shenshuaining Granules,and the observation group was added with Bailing Capsules and Shenshuaining Granules.The treatment lasted 16 weeks for both groups.The following indicators were observed and compared before and after treatment:micro-inflammatory indicators[high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),interleukin-8(IL-8),and tumor cell necrosis factor-α(TNF-α)],nutritional indicators[prealbumin(PA),transferrin(TRF),hemoglobin(Hb),and albumin(ALB)],T lymphocyte subsets[CD3^(+),CD4^(+),CD8^(+),and CD4^(+)/CD8^(+)],renal fibrosis indicators[serum transforming growth factor-β1(TGF-β1),type IV collagen(Col-IV),fibronectin(FN),and connective tissue growth factor(CTGF)],and renal function indicators[blood urea nitrogen(BUN),serum creatinine(SCr),24 h urine protein quantity(24 h UPQ),and estimated glomerular filtration rate(eGFR)].On this basis,the clinical efficacy was assessed.Results After treatment,the levels of micro-inflammatory indicators of the two groups all decreased as compared with those before treatment(P<0.05),and the levels in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of nutritional indicators showed no significant variation in the control group(P>0.05)and increased in the observation group(P<0.05).In addition,the levels of nutritional indicators in the observation were higher than those in the control group(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)increased and CD8^(+
