摘要
目的:观察百令胶囊联合坎地沙坦酯治疗糖尿病肾病(DN)的临床疗效。方法:采用随机数字表法将184例DN患者分为对照组和观察组各92例。对照组给予坎地沙坦酯片口服治疗,观察组在对照组基础上加用百令胶囊口服治疗。2组疗程均为8周。比较2组临床疗效,以及治疗前后中医证候积分、血糖、肾功能、血清壳多糖酶3样蛋白1(CHI3L1)、层粘连蛋白(LN)、Ⅳ型胶原(Ⅳ-C)水平。结果:观察组总有效率为92.39%,对照组为81.52%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组中医证候积分均较治疗前降低(P<0.05),且观察组中医证候积分低于对照组(P<0.05)。治疗后,2组空腹血糖、糖化血糖蛋白、血肌酐、血尿素氮、24 h尿蛋白定量、尿白蛋白排泄率均较治疗前降低(P<0.05),且观察组上述各项指标均低于对照组(P<0.05)。治疗后,2组血清CHI3L1、LN、Ⅳ-C水平均较治疗前降低(P<0.05),且观察组上述各项指标均低于对照组(P<0.05)。结论:百令胶囊联合坎地沙坦酯治疗DN疗效显著,可有效控制血糖,改善肾功能,降低中医证候积分和血清学纤维化指标,有效延缓肾纤维化进程。
Objective:To observe the clinical effect of Bailing Capsules combined with Candesartan Cilexetil for diabetic nephropathy(DN).Methods:A total of 184 cases of DN patients were divided into the control group and the observation group according to the random number table method,with 92 cases in each group.The control group was given the oral administration of Candesartan Cilexetil Tablets,and the observation group was additionally given the oral administration of Bailing Capsules based on the treatment of the control group.Both groups were treated for eight weeks.The traditional Chinese medicine(TCM)syndrome scores,and the levels of blood glucose,kidney function,and chitinase 3-like protein 1(CHI3L1),laminin(LN),and typeⅣcollagen(Ⅳ-C)in serum before and after treatment,as well as the clinical effects in the two groups were compared.Results:The total effective rate was 92.39%in the observation group and 81.52%in the control group,the difference being significant(P<0.05).After treatment,TCM syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and TCM syndrome score in the observation group was lower than that in the control group(P<0.05).After treatment,the levels of fasting blood glucose,glycated glycoprotein,serum creatinine,blood urea nitrogen,24-hour urinary protein quantification,and urinary albumin excretion rate in the two groups were decreased when compared with those before treatment(P<0.05),and the above indexes in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of CHI3L1,LN andⅣ-C in serum in the two groups were decreased when compared with those before treatment(P<0.05),and the above indexes in the observation group were lower than those in the control group(P<0.05).Conclusion:Bailing Capsules combined with Candesartan Cilexetil has a significant curative effect in the treatment of DN,which can effectively control blood glucose,improve kidney function,reduce TCM syndrome scores and serum fibrosis indexes
