摘要
目的:探究磁共振弥散加权成像评估骨肌肿瘤的价值。方法:选取2020年1月~2022年1月本院收治的骨肌肿瘤90例进行临床资料回顾性分析,所有患者均实施CT检查、磁共振弥散加权成像检查。以手术组织病理学检查结果为金标准,评价CT检查、磁共振弥散加权成像检查诊断效能,统计磁共振弥散加权成像检查中病变边缘、中央指数,分析良恶性骨肌肿瘤磁共振弥散加权成像表现情况。结果:90例入组患者,病理学检查结果显示良性骨肌肿瘤50例(50/90,55.56%),恶性骨肌肿瘤40例(40/90,44.44%);磁共振弥散加权成像检查灵敏度95.00%,特异度96.00%,准确度95.56%,均高于CT检查的75.00%、84.00%、80.00%,P<0.05;磁共振弥散加权成像检查中,病变边缘指数Iso、ADC良性骨肌肿瘤分别为(125.54±21.12)、(1.86±0.21)10-3mm2/s,高于恶性骨肌肿瘤(88.65±5.65)、(1.16±0.19)10-3mm2/s,P<0.05;病变边缘指数FA、VrA良性骨肌肿瘤患者分别为(0.14±0.01)、(0.01±0.001),低于恶性骨肌肿瘤的(0.22±0.02)、(0.02±0.002),P<0.05;病变中央指数Iso良性骨肌肿瘤(138.98±22.65)、ADC良性骨肌肿瘤(1.98±0.21)10-3mm2/s,高于恶性骨肌肿瘤(115.58±25.26)、(1.54±0.25)10-3mm2/s,P<0.05;磁共振弥散加权成像检查中恶性骨肌肿瘤瘤周水肿率40.00%(16/40)、液化坏死率75.00%(30/40)高于良性骨肌肿瘤的8.00%(4/50)、10.00%(5/50),P<0.05。结论:磁共振弥散加权成像能够有效评估骨肌肿瘤良恶性,其在骨肌肿瘤诊断中的灵敏度、特异度、准确度较高,可为临床治疗提供较为全面且准确的检查信息。
Objective:To explore the value of diffusion-weighted magnetic resonance imaging(DWI)in the evaluation of osteomuscular tumors.Methods:The clinical data of 90 cases of osteomuscular tumors treated in our hospital from January 2020 to January 2022 were retrospectively analyzed.All patients underwent CT and diffusion weighted magnetic resonance imaging.Taking the results of surgical histopathology as the gold standard,the diagnostic efficacy of CT and DWI was evaluated,the lesion edge and central index in DWI were counted,and the MRI findings of benign and malignant bone and muscle tumors were analyzed.Results:Among the 90 patients,50 cases(50/90,55.56%)were benign osteomuscular tumors and 40 cases(40/90,44.44%)were malignant osteomuscular tumors;The sensitivity,specificity and accuracy of DWI were 95.00%,96.00%and 95.56%respectively,which were higher than those of CT 75.00%,84.00%and 80.00%(P<0.05);During the MR diffusion-weighted imaging examination,the lesion margin index Iso and ADC were(125.54±21.12),(1.86±0.21)10-3mm2/s,higher than the malignant bone muscle tumors(88.65±5.65),(1.16±0.19)10-3mm2/s,P<0.05;Patients with the lesion margin index FA and VrA were(0.14±0.01)and(0.01±0.001),respectively,below the malignant bone muscle tumor(0.22±0.02),(0.02±0.002)(P<0.05);Iso benign benign(138.98±22.65),ADC benign(1.98±0.21)10-3mm2/s,higher than the malignant bone muscle tumors(115.58±25.26),(1.54±0.25)10-3mm2/s(P<0.05);In MR diffusion-weighted imaging examination,the rate of malignant bone muscle tumor peritumor edema was 40.00%(16/40),and the rate of liquefied necrosis was 75.00%(30/40),which was higher than 8.00%(4/50)and 10.00%(5/50)of benign bone muscle tumors(P<0.05).Conclusion:Diffusion weighted MR imaging can effectively evaluate the benign and malignant bone and muscle tumors.It has high sensitivity,specificity and accuracy in the diagnosis of bone and muscle tumors,and can provide more comprehensive and accurate examination information for clinical treatment.
