摘要
目的:探究磁共振弥散加权成像(diffusion weighted imaging,DWI)联合3D动脉自动自旋标记技术(3-dimensional arterial spin labeling,3D ASL)对急性缺血性脑卒中(acute ischemic stroke,AIS)的诊断价值。方法:选取福建医科大学附属福州市第一医院2018年8月-2020年8月收治的48例急性缺血性脑卒中(发病<3 d)患者作为研究对象,同时选取48例健康人群作为对照组。采用DWI联合3D ASL技术测量脑梗死区、缺血半暗带及其镜面区的表观扩散系数(apparent diffusion coefficient,ADC)和脑血流量(cerebral blood flow,CBF)绝对值和相对值,计算相对CBF(rCBF)和相对ADC(rADC),比较DWI联合3D ASL对AIS的诊断价值。结果:ASI患者脑梗死病灶区rCBF为(26.16±8.23)ml/(min·100 g),缺血半暗带区的rCBF为(35.35±4.16)ml/(min·100 g),镜面区rCBF为(46.03±6.28)ml/(min·100 g),与对照组比较差异均有统计学意义(P<0.05)。ASI患者脑梗死病灶区的rADC为(0.46±0.16)μm^(2)/ms,缺血半暗带区的rADC为(0.65±0.14)μm^(2)/ms,镜面区rADC为(0.79±0.11)μm^(2)/ms,脑梗死病灶区、缺血半暗带rADC与对照组比较均显著较低(P<0.05)。DWI联合3D ASL检出ASI患者出血敏感度为87.50%,准确度为85.42%。结论:DWI联合3D-ASL检测有助于提高ASI诊断的准确性,对后续治疗及康复具有重要指导价值。
Objective:To explore the diagnostic value of diffusion-weighted imaging(DWI)combined with 3-dimensional arterial spin labeling(3D ASL)for the acute ischemic stroke(AIS).Method:A total of 48 patients with AIS(onset<3 d)in Fuzhou First Hospital,Fujian Medical University from August 2018 to August 2020 were selected as the research objects,and 48 cases of healthy people as control group.DWI combined 3D ASL were used to measure the absolute,relative apparent diffusion coefficient(ADC)and cerebral blood flow(CBF)of cerebral infarction area,ischemia half dark zone and its mirror area.The absolute value and relative value of rCBF and rADC were evaluated to observe the diagnostic value of DWI combined 3D ASL for the AIS.Result:The rCBF of the cerebral infarction lesions in ASI patients was(26.16±8.23)ml/(min·100 g),and the rCBF of the ischemia penumbra was(35.35±4.16)ml/(min·100 g),the rCBF of the mirror area was(46.03±6.28)ml/(min·100 g),the differences were statistically significant compared with the control group(P<0.05).Compared with the control group,the rADC in ASI patients was(0.46±0.16)μm^(2)/ms,the rADC in the ischemic penumbra area was(0.65±0.14)μm^(2)/ms,and the rADC in the mirror area was(0.79±0.11)μm^(2)/ms,rADC of cerebral infarction lesions area,ischemic penumbra area were significantly lower than that in control group(P<0.05).DWI combined with 3D ASL could check out the ASI bleeding in patients with the sensitivity of 87.50%and the accuracy of 85.42%.Conclusion:The detection of DWI combined with 3D-ASL is helpful to improve the accuracy of ASI diagnosis,which has important guidance value for subsequent treatment and rehabilitation.
