摘要
目的:探讨中药熏蒸与止痒息风汤加减内服治疗糖尿病皮肤瘙痒的疗效及对患者血清人干扰素γ(IFN-γ)、白细胞介素2(IL-2)、肿瘤坏死因子α(TNF-α)、免疫球蛋白E(IgE)的影响。方法:采用简单随机抽样法,将84例糖尿病皮肤瘙痒患者分为观察组和对照组,各42例。对照组给予常规治疗(基础降糖+氯雷他定),观察组在常规治疗基础上给予中药熏蒸联合止痒息风汤治疗。观察两组临床疗效,治疗前后临床症状积分、血清IFN-γ、IL-2、TNF-α及IgE的比较,不良反应的发生情况。结果:观察组治疗总有效率(90.48%)高于对照组(69.05%)(P<0.05)。治疗后,观察组患者各项临床症状积分、血清IFN-γ、IL-2、TNF-α、IgE水平均低于对照组(P<0.05)。观察组不良反应发生率7.14%低于对照组的19.04%(P>0.05)。结论:中药熏蒸与止痒息风汤加减内服治疗糖尿病皮肤瘙痒疗效确切,可明显改善皮肤瘙痒症状,下调血清IFN-γ、IL-2、TNF-α及IgE水平,且不良反应少。
Objective:To investigate the effect of Chinese herbal fumigation and Zhiyang Xifeng decoction on diabetic skin pruritus and the influence on serum levels of human interferon-γ(IFN-γ),interleukin-2(IL-2),tumor necrosis factor-α(TNF-α)and immunoglobulin E(IgE)in patients with diabetic skin pruritus.Methods:Using simple random sampling method:84 cases of diabetic skin pruritus were divided into observation group and control group,42 cases in each group.The control group was given conventional treatment(basic hypoglycemic+loratadine),and the observation group was given traditional Chinese medicine fumigation combined with Zhiyang Xifeng decoction on the basis of conventional treatment.The clinical efficacy,clinical symptom score,serum IFN-γ,IL-2,TNF-αand IgE before and after treatment were observed.Results:The total effective rate of the observation group(90.48%)was higher than that of the control group(69.05%)(P<0.05).After treatment,the clinical symptom scores and serum IFN-γ、IL-2、TNF-α、the level of IgE in the observation group was lower than that in the control group(P<0.05).The incidence of adverse reactions in the observation group(7.14%)was lower than that in the control group(19.04%)(P>0.05).Conclusion:Chinese herbal fumigation and Zhiyang Xifeng decoction have definite curative effect on diabetic skin pruritus.It can significantly improve the symptoms of skin itching,and lower the levels of serum IFN-γ,IL-2,TNF-αand IgE,with less adverse reactions.
