摘要
目的:探讨老年腰椎间盘突出症(LDH)患者给予经椎间孔入路脊柱内镜手术对临床疗效、腰椎功能及生活质量的影响。方法:选取2020年6月-2021年12月景德镇市第二人民医院收治的86例老年LDH患者,按照随机数字表法分成观察组(43例)与对照组(43例)。对照组给予后路椎板减压植骨融合椎弓根螺钉内固定术,观察组给予经椎间孔入路脊柱内镜手术。比较两组临床疗效、腰椎功能、生活质量。结果:观察组(86.05%)与对照组(65.12%)相比治疗总优良率更高(P<0.05)。术后3个月,两组腰椎日本骨科协会评估治疗分数(JOA)评分均较术前提高,Oswestry功能障碍指数(ODI)均较术前降低,与对照组相比,观察组腰椎JOA评分更高,ODI更低(P<0.05)。术后3个月,两组各维度WHOQOL-BREF评分均较术前提高,与对照组相比,观察组评分均更高(P<0.05)。结论:老年LDH患者给予经椎间孔入路脊柱内镜手术,能够提升临床疗效,改善患者腰椎功能,提高生活质量。
Objective:To investigate the effect of endoscopic spinal surgery via intervertebral foramen on clinical efficacy,lumbar function and quality of life in elderly patients with lumbar disc herniation(LDH).Method:A total of 86 elderly patients with LDH treated in the Second People’s Hospital of Jingdezhen from June 2020 to December 2021 were selected,they were divided into the observation group(43 cases)and the control group(43 cases)according to random number table.The control group was treated with posterior laminectomy,bone grafting and pedicle screw internal fixation,and the observation group was treated with endoscopic spinal surgery via intervertebral foramen.The clinical efficacy,lumbar function and quality of life were compared between the two groups.Result:The total excellent and good rate(86.05%)in the observation group was higher than 65.12%in the control group(P<0.05).3 months after surgery,the Japanese orthopaedic association assessment and treatment score(JOA)scores of lumbar spine in both groups were higher than those before surgery,and the Oswestry disability index(ODI)in both groups were lower than those before surgery,compared with the control group,the JOA score of lumbar spine in the observation group was higher and the ODI was lower(P<0.05).3 months after surgery,the WHOQOL-BREF scores of each dimension in both groups were higher than those before surgery,and those in the observation group were higher than those in the control group(P<0.05).Conclusion:Endoscopic spinal surgery via intervertebral foramen in elderly patients with LDH can improve clinical efficacy,improve lumbar function and improve quality of life.
