摘要
目的:探讨复合麻醉联合经椎间孔内镜脊柱系统(TESSYS)技术治疗腰椎间盘突出症(LDH)的效果。方法:选取2023年1—12月汕尾市人民医院收治的LDH患者62例为研究对象,按照随机抽样法分为观察组和对照组,各31例。两组均采用复合麻醉,对照组采用椎板开窗髓核摘除术治疗,观察组采用TESSYS技术治疗。比较两组手术指标、疼痛介质、疼痛程度、腰椎功能、并发症。结果:观察组出血量低于对照组,手术时间及住院时间短于对照组,差异有统计学意义(P<0.001);术后7 d,两组前列腺素E2、P物质、神经肽水平降低,观察组低于对照组,差异有统计学意义(P<0.05);术后7 d,两组视觉模拟量表、Oswestry功能障碍指数评分降低,观察组低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P=0.037)。结论:复合麻醉联合TESSYS技术治疗LDH可减轻对机体的创伤,改善患者的腰椎功能,降低疼痛介质水平,且安全性较高。
Objective:To explore the effect of combined anesthesia and transforaminal endoscopic spinal system(TESSYS)tech⁃nology in the treatment of lumbar disc herniation(LDH).Methods:Sixty-two LDH patients admitted to Shanwei People's Hospital from January to December 2023 were selected as the research subjects and divided into an observation group and a control group according to the random sampling method,with 31 cases in each.Both groups were given combined anesthesia,the control group was treated with laminectomy and the observation group was treated with TESSYS technology.The surgical indicators,pain media,pain levels,lumbar spine function and complications were compared between the two groups.Results:The bleeding volume of the observation group were lower than those of the control group,and the operation time and hospitalization time were shorter than those of the control group,the differences were statistically significant(P<0.001);on the 7th day after surgery,the levels of prosta⁃glandin E2,substance P,and neuropeptides in the two groups were reduced,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05);on the 7th day after surgery,the Visual Analog Scale and Oswes⁃try Disability Index scores of the two groups decreased,and the observation group was lower than the control group,and the dif⁃ference was statistically significant(P<0.05);the incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.037).Conclusion:The treatment of LDH with combined anes⁃thesia and TESSYS technology can reduce the trauma to the body,improve the patient's lumbar spine function,reduce the level of pain mediators,and is highly safe.
