摘要目的观察加味桔芩汤对支气管扩张痰热壅肺证患者抑郁焦虑、生命质量、炎症标记物及肺功能的疗效。方法81例支气管扩张慢性持续期痰热壅肺证患者,随机分成对照组40例和干预组41例。2组均采用西医常规治疗,干预组加用中药加味桔芩汤治疗。2组均治疗4周后比较抑郁焦虑、生命质量、炎症标记物及肺功能的变化。结果干预组汉密顿抑郁评分表(HAMD)、汉密顿焦虑评分表(HAMA)、抑郁自评分表(SDS)、焦虑自评分表(SAS)评分较治疗前显著降低(P<0.05)。2组患者的肺部症状、疾病影响、活动限制评分和SGRQ总评分较治疗前明显降低,且干预组显著低于对照组(P<0.05)。2组患者血清降钙素原(PCT)、C反应蛋白(CRP)、白细胞计数(WBC)和白细胞介素6(IL-6)水平均较治疗前明显下降,且干预组显著低于对照组(P<0.05)。2组患者第1秒用力呼气容积(FEVl)、一秒率(FEVl/FVC)、用力肺活量(FVC)水平均较治疗前明显提高,且干预组显著高于对照组(P<0.05)。2组一氧化碳弥散量(DLCO)水平均无改善(P>0.05)。结论加味桔芩汤治疗支气管扩张痰热壅肺证效果显著,可显著缓解患者抑郁焦虑情绪、提高生命质量、降低炎症标记物水平并改善肺通气功能。 Objective To observe the effect of the modified Jieqin Decoction on the depression and anxiety,quality of life,infl ammatory markers and pulmonary function in bronchiectasis patients with the syndrome of phlegm-heat obstructing the lung.Methods A total of 81 patients of chronic persistent bronchiectasis with the syndrome of phlegmheat obstructing the lung were randomly divided into a control group(n=40)and an intervention group(n=41).Both groups were treated with routine western medicine,while the intervention group was additionally treated with the modified Jieqin Decoction.The changes in depression and anxiety,quality of life,inflammatory markers and pulmonary function were compared in both groups after 4 weeks of treatment.Results After 4 weeks of treatment,the scores of the Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA),Self-Rating Depression Scale(SDS)and Self-Rating Anxiety Scale(SAS)were significantly lower than before treatment only in the intervention group(P<0.05).The scores of lung symptoms,disease effects,activity restriction and the total scores of St.George’s Respiratory Questionnaire(SGRQ)were significantly lower than before treatment in the two groups,and the intervention group was significantly lower than the control group(P<0.05).The serum levels of procalcitonin(PCT),C-reactive protein(CRP),white blood cell count(WBC),interleukin 6(IL-6)were signifi cantly lower than before treatment in the two groups,and the intervention group was signifi cantly lower than the control group(P<0.05).The forced expiratory volume in the first second(FEVl),one second rate(FEVl/FVC),forced vital capacity(FVC)were significantly improved compared to before treatment in the two groups,and the intervention group was significantly higher than the control group(P<0.05).There was no significant improvement in the DLCO level in the two groups(P>0.05).Conclusion The modified Jieqin Decoction is eff ective in the treatment of bronchiectasis with the syndrome of phlegm-heat obstructing the lung,which can signi
