摘要
目的探讨多系统萎缩(MSA)患者认知损害的危险因素。方法共纳入100例MSA患者,根据蒙特利尔认知评估量表(MoCA)评分分为认知功能正常组(≥26分)27例、轻度认知障碍组(22~25分)35例、重度认知障碍组(≤21分)38例。采用多因素有序Logistic回归筛选影响MSA患者认知功能的危险因素。结果有序Logistic回归模型1纳入入组年龄和其他因变量显示,认知水平与入组年龄(OR=1.109,95%CI:1.037~1.187,P=0.003)、受教育年限(OR=0.770,95%CI:0.666~0.890,P<0.001)相关,已戒酒组MSA患者(OR=0.239,95%CI:0.064~0.885,P=0.032)出现认知损害的风险较仍在饮酒组低。模型2纳入发病年龄分析显示,认知水平与发病年龄(OR=1.115,95%CI:1.043~1.192,P=0.001)、受教育年限(OR=0.782,95%CI:0.677~0.904,P=0.001)相关,已戒酒组MSA患者(OR=0.230,95%CI:0.062~0.858,P=0.029)出现认知损害的风险较仍在饮酒组低。结论MSA患者认知损害是多因素共同作用的结果,其中年龄是独立危险因素,受教育年限是独立保护因素,饮酒可能是其潜在的危险因素。
Objective To investigate the risk factors of cognitive impairment in patients with multiple system atrophy(MSA).Methods A total of 100 patients with MSA were divided into the normal cognitive function group(≥26 points,27 cases),the mild cognitive impairment group(22-25 points,35 cases)and the severe cognitive impairment group(≤21 points,38 cases)according to the score of the Montreal cognitive assessment scale(MoCA).Multivariate ordinal Logistic regression analysis was used to screen the related risk factors affecting cognitive function in patients with MSA.Results Age at admission and other variables were included in Model 1,the results showed that cognitive function was related with age at admission(OR=1.109,95%CI:1.037-1.187,P=0.003)and years of education(OR=0.770,95%CI:0.666-0.890,P<0.001),the risk of cognitive impairment of former drinking is less than current drinking(OR=0.239,95%CI:0.064-0.885,P=0.032).Age at onset and other variables were included in Model 2,the results showed that cognitive function was related with age at onset(OR=1.115,95%CI:1.043-1.192,P=0.001)and years of education(OR=0.782,95%CI:0.677-0.904,P=0.001),the risk of cognitive impairment of former drinking was less than current drinking(OR=0.230,95%CI:0.062-0.858,P=0.029).Conclusion Impairment of global cognitive function in patients with MSA correlates with multiple factors,in which age is an independent risk factor,years of education is independent protective factor,and drinking may be a potential risk factor.
