摘要
目的分析老年患者ICU后综合征发生现状及症状特征分布特点,为了解老年患者ICU后综合征及制定早期护理干预方案提供参考。方法采用便利抽样法选取2019年8月至2020年9月某三级甲等医院综合ICU老年住院患者为研究对象,采用简易精神状态量表、住院焦虑抑郁量表、事件影响量表修订版、Barthel指数评定量表、匹兹堡睡眠质量指数量表及疲劳评定量表,在患者转出ICU后1个月对其进行认知、心理、生理3个方面的评估,并采用患者一般资料和疾病相关资料对ICU后综合征的影响因素进行分析。结果 103例ICU转出老年患者中,78例发生ICU后综合征,发生率为75.73%。Logistic回归分析显示,老年患者发生ICU后综合征的影响因素为吸烟/酗酒、机械通气时长、ICU住院时长(P<0.05)。结论老年患者ICU后综合征的发生率高,吸烟/酗酒、机械通气时间长及ICU住院时间长是导致老年患者发生ICU后综合征的危险因素。医护人员应重视老年患者ICU综合征,在住ICU期间及转出ICU后早期采取干预措施,降低老年患者ICU后综合征的发生。
Objective To analyze the present situation of post-ICU syndrome in elderly patients for the understanding of the elderly patients after ICU syndrome and the formulation of early nursing intervention program to provide reference.Methods A convenient sampling method was used to select elderly inpatients of our hospital from August 2019 to September 2020.Mini-mental State Examination, Hospital Anxiety and Depression Scale, Impact of Events Scale-Revised, Barthel index, Pittsburgh Sleep Quality Index, and Fatigue Assessment Instrument were used to assess the patients’ cognitive, psychological, physiological status, and the influencing factors of post-ICU syndrome were analyzed by general data and disease related data.Results Among 103 elderly, 78(75.73%) had post-ICU syndrome.Logistic regression analysis showed the influencing factors of post-ICU syndrome in elderly patients were smoking/alcohol abuse, long period of mechanical ventilation and the length of ICU stay(P<0.05).Conclusion The incidence of post-ICU syndrome in elderly patients is high, smoking/alcohol abuse, long-term of mechanical ventilation and ICU length of stay are risk factors for post-ICU syndrome in elderly patients.Medical staff should pay attention to post-ICU syndrome in older patients, and take interventions during ICU residence and take early measures after transfer ring to ICU to reduce the occurrence of syndrome after ICU.
