摘要
目的:探讨基线^(18)F-脱氧葡萄糖(FDG) PET/CT代谢参数在神经母细胞瘤(NB)患儿预后评估中的价值。方法:收集2013年10月至2019年10月病理确诊为NB于我院行治疗前^(18)F-FDG PET/CT检查的37例患者影像和临床资料,并统计2年无进展生存期(PFS)和总生存期(OS)。采用受试者工作特征曲线(ROC)分析确定各参数的截断值。采用单因素Kaplan-Meier法及多因素Cox回归分析评估各参数对PFS和OS的影响。结果:单因素分析显示:血清乳酸脱氢酶(LDH)、血清神经元特异性烯醇化酶(NSE)、血清铁蛋白、原发肿瘤最大径、原发肿瘤最大标准化摄取值(Pmax)、所有肿瘤最大标准化摄取值(Tmax)、Tmax与肝脏平均标准化摄取值的比值(Tmax/Lmean)、原发肿瘤的糖酵解总量(MTV)及糖酵解总量(TLG)是影响PFS的因素;血清LDH、血清NSE、原发肿瘤最大径、Pmax、Tmax/Lmean、MTV及TLG是影响OS的因素。多因素分析显示:血清LDH、血清铁蛋白、Tmax/Lmean、MTV是PFS的独立预后因素,MTV是OS的独立预后因素。结论:MTV是PFS和OS的唯一独立预后因素。^(18)F-FDG PET/CT能为NB患儿提供可靠的预后信息。
Purpose:To evaluate the prognostic value of metabolic parameters measured by pretherapeutic ^(18)F-deoxyglucose(FDG)positron emission tomography/computed tomography(PET/CT)in pediatric patients with neuroblastoma(NB).Methods:The clinical and image data of 37 patients with NB diagnosed by pathology results in our hospital from October 2013 to October 2019 who underwent^(18)F-FDG PET/CT were retrospectively reviewed,and the 2-year progression-free survival(PFS)and overall survival(OS)were counted.The cutoff values of all parameters were determined by receiver operating characteristic(ROC)curve analysis.Univariate Kaplan-Meier analysis and multivariate Cox proportional hazard regression analysis were used to evaluate the impact factors of PFS and OS in pediatric patients with neuroblastoma.Results:Univariate Kaplan-Meier analysis showed that serum lactate dehydrogenase(LDH),serum neuron specific enolase(NSE),serum ferritin,maximum diameter of primary tumor,the maximum standardized uptake value of primary tumor lesions(Pmax),the maximum standardized uptake value of all tumor lesions(Tmax),the ratio of Tmax to[average standardized uptake value of liver tissue(Lmean)](Tmax/Lmean),metabolic volume of primary tumor(MTV)and total lesion glycolysis of primary tumor(TLG)were the prognostic factors for PFS,while serum LDH,serum NSE,maximum diameter of primary tumor,Pmax,Tmax/Lmean,MTV and TLG were the prognostic factors for OS.Multivariate Cox proportional hazard regression analysis revealed that serum LDH,serum ferritin,Tmax/Lmean and MTV were the independent prognostic factors for PFS,while MTV was the independent prognostic factor for OS.Conclusions:MTV measured by PET/CT was the only independent prognostic factor for PFS and OS.Pretherapeutic^(18)F-FDG PET/CT can provide reliable prognostic information for pediatric patients with neuroblastoma.
