摘要
目的:探讨甲状腺腺脂肪瘤的临床病理特征、诊断及鉴别诊断。方法:收集2021年7月西南医科大学附属医院病理科诊断的1例甲状腺腺脂肪瘤伴周围甲状腺组织局灶结节性甲状腺肿的临床及影像学资料,通过观察组织学形态和免疫组化表型分析其临床病理特征,并进行相关文献复习。结果:患者女,病变位于甲状腺左叶,病理学诊断为甲状腺腺脂肪瘤,大体表现为灰黄色包块,被覆完整包膜,切面呈灰黄灰红色,略带油腻感;镜下见滤泡性腺瘤中夹杂有成熟的脂肪细胞,脂肪细胞占肿瘤细胞的比例约为10%;另见一灰黄色结节,镜下呈结节性甲状腺肿改变。免疫表型:PAX8、TG、TTF-1均阳性、CD56弥漫阳性、Cam5.2局灶阳性、Ki-67阳性指数<1%、p53<1%;Syn、CgA、GATA3均阴性。结论:甲状腺腺脂肪瘤是发生于甲状腺的罕见良性肿瘤,生长缓慢,患者常无明显临床症状,超声征象无特异性,易误诊,明确诊断常依靠术后病理。
Objective:To investigate the clinicopathological features,diagnosis and differential diagnosis of adenolipoma of the thyroid gland.Methods:Clinical and imaging data of a case of adenolipoma of the thyroid gland with peripheral thyroid focal nodular goiter diagnosed in the pathology department of the Affiliated Hospital of Southwest Medical University in July 2021 were collected.The clinicopathological features were analyzed by observing the histological morphology and immunohistochemical phenotype,and literature relevant to adenolipoma of the thyroid gland was reviewed.Results:The patient was a female with the lesion located in the left lobe of the thyroid gland,and was pathologically diagnosed as thyroid gland adenolipoma with nodular goiter.The gross appearance was grayish yellow mass covered with complete capsule.The section was grayish yellow and grayish red,and was slightly greasy.Microscopically,the follicular adenoma was mixed with mature adipocytes,accounting for about 10%of the tumor cells;and there was another grayish yellow nodule with nodular goiter.Immunohistochemistry showed that Pax8,TG and TTF-1 were positive;CD56 was diffuse positive in tumor cells,and Cam5.2 was focal positive;Ki67 and p53 were less than 1%;Syn,CgA and GATA3 were all negative.Conclusion:Adenolipoma of the thyroid gland,which grows slowly,is a rare benign tumor of thyroid gland.It is easy to be misdiagnosed for barely obvious clinical symptoms and nonspecific ultrasonic signs.It can be further confirmed by postoperative pathology.
