妊娠糖尿病患者血清HbA1c、hs-CRP的表达及其与新生儿低血糖发生的关系 被引量:2

Expression of Serum HbA1c and hs-CRP in Patients with Gestational Diabetes Mellitus and Its Relationship with Neonatal Hypoglycemia

摘要 目的观察妊娠糖尿病(GDM)患者血清糖化血红蛋白(HbA1c)、超敏C反应蛋白(hs-CRP)的表达及新生儿低血糖发生情况,并分析血清HbA1c、hs-CRP的表达与新生儿低血糖发生的关系。方法选取2018年4月至2020年10月信阳市妇幼保健院收治的112例GDM患者,检测患者血清HbA1c、hs-CRP水平,以GDM患者分娩后新生儿是否发生低血糖进行分组,分为发生组与未发生组。设计基线资料调查表,比较患者基线资料,分析GDM患者血清HbA1c、hs-CRP表达与新生儿低血糖发生的关系,并基于受试者工作特征(ROC)曲线分析GDM患者HbA1c、hs-CRP表达预测新生儿低血糖发生的价值。结果112例GDM患者分娩后,26例(23.21%)新生儿发生低血糖,86例(76.79%)未发生;两组年龄、分娩孕周、产前体质量指数、分娩方式、孕期使用胰岛素、使用分娩镇痛比较,差异无统计学意义(P>0.05);发生组血清HbA1c、hs-CRP水平及血糖控制欠佳占比高于未发生组(P<0.05);logistic回归分析结果显示,GDM患者血清HbA1c、hs-CRP高表达以及孕期血糖控制欠佳是新生儿低血糖发生的危险因子(OR>1,P<0.05);ROC曲线结果显示,血清HbA1c、hs-CRP表达单独及联合预测GDM患者新生儿低血糖发生风险的曲线下面积(AUC)分别为0.801、0.770、0.913,均>0.7,具有一定的预测价值,且联合预测价值较高。结论GDM患者血清HbA1c、hs-CRP表达与新生儿低血糖的发生密切相关。 Objective To observe the expression of serum glycosylated hemoglobin(HbA1c),high sensitivity C reactive protein(hs-CRP)and the occurrence of neonatal hypoglycemia in patients with gestational diabetes mellitus(GDM),and to analyze the relationship between the expression of serum HbA1c and hs-CRP and the occurrence of neonatal hypoglycemia.Methods A total of 112 patients with GDM treated in Xinyang Maternal and Child Health Care Hospital from April 2018 to October 2020 were selected.The levels of serum HbA1c and hs-CRP were detected.According to whether the newborns of GDM patients had hypoglycemia after delivery,they were divided into occurrence group and non occurrence group.Designed the baseline data questionnaire,compared the baseline data of patients,analyzed the relationship between the expression of serum HbA1c and hs-CRP in GDM patients and the occurrence of neonatal hypoglycemia,and analyzed the value of the expression of HbA1c and hs-CRP in GDM patients in predicting the occurrence of neonatal hypoglycemia based on the receiver operating characteristic(ROC)curve.Results After delivery,26 cases(23.21%)of 112 patients with GDM had hypoglycemia,and 86 cases(76.79%)did not have hypoglycemia.There was no statistical difference in age,gestational weeks,prenatal body mass index,mode of delivery,insulin use during pregnancy and labor analgesia between the two groups(P>0.05).The levels of serum HbA1c,hs-CRP and the proportion of poor blood glucose control in the occurrence group were higher than those in the non occurrence group(P<0.05).The results of logistic regression analysis showed that the high expression of serum HbA1c and hs-CRP and poor control of blood glucose during pregnancy were the risk factors of neonatal hypoglycemia(OR>1,P<0.05).ROC curve results showed that,the area under the curve(AUC)of serum HbA1c and hs-CRP expression alone and combined to predict the risk of neonatal hypoglycemia in patients with GDM were 0.801,0.770 and 0.913,all of which were>0.7,with certain prediction value,and the joint
