摘要
目的 研究血清白介素-1β(IL-1β)、CD64指数及血清淀粉样蛋白A(SAA)水平与白内障术后眼内炎的关系。方法 选取2019年1月至2021年1月于保定市第一医院行手术治疗的103例白内障为研究组,另选取同期于本院健康体检的正常人群88例为对照组。比较两组IL-1β、CD64、SAA;统计白内障患者术后感染眼内炎率,再分感染组与未感染组2个亚组,并进行IL-1β、CD64、SAA比较,以logistic分析影响白内障患者感染眼内炎的危险因素。结果 研究组IL-1β、CD64、SAA高于对照组,差异均有统计学意义(P<0.05)。经统计,术后感染患者为22例。感染组IL-1β、CD64、SAA高于未感染组,差异均有统计学意义(P<0.05)。年龄、吸烟、高血压、糖尿病、手术时间、玻璃体溢出、IL-1β、CD64、SAA为影响患者感染眼内炎的单因素(P<0.05)。年龄、手术时间、玻璃体溢出、IL-1β、CD64、SAA为影响白内障患者感染眼内炎的独立危险因素(P<0.05)。结论 IL-1β、CD64、SAA在白内障术后眼内炎患者机体中异常升高,通过检测三者可对白内障术后患者是否感染眼内炎进行有效评估。
Objective To study the relationship between serum interleukin-1 β(IL-1β),CD64index and serum amyloid A protein(SAA)level and endophthalmitis after cataract surgery. Methods 103cataract patients who underwent surgery in Baoding First Hospital from January 2019 to January 2021 were selected as the study group,and 88 normal people who underwent physical examination in our hospital during the same period were selected as the control group. The IL-1β,CD64 and SAA in the two groups were compared;the incidence of postoperative endophthalmitis infection in cataract patients was counted,and the patients were divided into two subgroups,the infection group and the uninfected group,and IL-1β,CD64 and SAA were compared,and the risk factors of endophthalmitis infection in cataract patients were analyzed by logistic analysis. Results The levels of IL-1β,CD64 and SAA in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05). According to the statistics,there were22 cases of postoperative infection. The levels of IL-1β,CD64 and SAA in the infected group were higher than those in the uninfected group,and the differences were statistically significant(P<0.05). Age,smoking,hypertension,diabetes,operation time,vitreous overflow,IL-1β,CD64 and SAA were independent risk factors for endophthalmitis infection in cataract patients(P<0.05). Conclusion The levels of IL-1β,CD64 and SAA are abnormally elevated in patients with endophthalmitis after cataract surgery,and the detection of these three can be used to effectively evaluate the infection of endophthalmitis after cataract surgery.
