摘要
目的:观察温胆汤、左金丸、四逆散配伍加减结合泮托拉唑钠肠溶片口服治疗反流性食管炎的临床效果。方法:反流性食管炎多由饮食不节、劳倦过度、情志失调所致,与痰热犯胃,脾胃升降异常有关。故清热化痰,和胃降逆为治疗关键。采用温胆汤、左金丸、四逆散配伍,并随证加减治疗35例反流性食管炎患者:若呕吐去柴胡;脾虚者加太子参、白术;肝胃郁热加黄芩、金银花;吐酸加煅瓦楞子;阴虚加麦冬;痛甚加香附、延胡索;血瘀加丹参;食滞加炒谷芽、炒麦芽。并合用泮托拉唑钠肠溶片,每天40 mg口服,以4周为一个疗程。结果:治疗组总有效率优于对照组。结论:清热化痰、和胃降逆法组方可有效缓解反流性食管炎之烧心、反酸、胃痛等证,有利于食管炎症愈合。温胆汤、左金丸、四逆散配伍加减能明显提高反流性食管炎的治疗效果。
Objective:To observe the clinical effect of compatibility of the Wendan decoction(温胆汤),Zuojin pill(左金丸)and Sini powder(四逆散),combined with oral pantoprazole sodium enteric-coated tablet on reflux esophagitis.Methods:Most of reflux esophagitis is caused by undisciplined diet,excessive fatigue and emotional disorders.It’s associates with phlegm and heat invasion of the stomach and abnormal spleen and stomach function.Therefore,clearing heat and reducing phlegm and stomach-regulating and Qi-lowering(和胃降逆)are the key points of treatment.A total of 35 cases were treated with compatibility of the Wendan decoction,Zuojin pill and Sini powder.If the patient vomits,Chaihu(Bupleurum)in the formula is removed.For patients with spleen deficiency,Taizishen(Radix pseudostellariae)and Baizhu(Atractylodes)are included in the prescription.If the patient's liver and stomach are stagnant and heat,Huangqin(Skullcap)and Jinyinhua(Honeysuckle)are included in the prescription.If the patient vomits acid,Duan Walengzi(Calcined arca subcrenata)are included in the prescription.For patients with Yin(阴)deficiency,Maidong(Ophiopogon japonicus)is included in the prescription.If the patient is in severe pain,Xiangfu(Rhizoma cyperi)and Yanhusuo(Corydalis yanhusuo)are included in the prescription.If the patient has blood stasis,Danshen(Salvia miltiorrhiza)is included in the prescription.If the patient suffers from food stagnation,Chao Guya(Fried grain sprouts)and Chao Maiya(Fried malt)are included in the prescription.Pantoprazole sodium enteric-coated tablets were used in combination,40 mg orally per day for 4 weeks as a course of treatment.Results:The total effective rate in the treatment group was better than the control group.Conclusion:It showed that the formula of clearing heat and reducing phlegm and stomach-regulating and Qi-lowering could effectively relieve heartburn,acid reflux and stomachache,and heal the inflammation of the esophagus.The compatibility of the Wendan decoction,Zuojin pill and Sini powder cou
