摘要
目的探讨在妇科手术麻醉期间应用喉罩全麻通气的实用性和安全性。方法选取包钢集团第三职工医院2018年1月-2020年12月妇科手术患者80例;随机分为气管插管组和喉罩组,各40例;记录两组患者一次放置成功率、血流动力学指标及并发症发生率。结果喉罩组一次放置成功率(97.50%)高于气管插管组(75.00%),差异有统计学意义(χ^(2)=8.538,P<0.05);治疗后,喉罩组血流动力学各项指标中舒张压(77.11±6.25)mmHg、收缩压(122.35±9.13)mmHg、心率(86.39±10.29)次/min、平均动脉压(83.35±7.49)mmHg均低于气管插管组,差异有统计学意义(P<0.05);喉罩组并发症总发生率(10.00%)明显低于气管插管组30.00%,差异有统计学意义(P<0.05)。结论喉罩通气应用妇科手术时,患者一次放置成功率高,应激反应小,并能显著降低并发症发生率,整体提高患者预后水平。
Objective To investigate the practicability and safety of applying laryngeal mask ventilation under general anesthesia during gynecological surgery.Methods A total of 80 patients with elective gynecological surgery from January 2018 to December 2020 were selected and randomly divided into tracheal intubation group and laryngeal mask group,40 cases in each group.The success rate of one-time placement,hemodynamic indexes and complication rates of the two groups were recorded.Results The success rate of one-time placement in the laryngeal mask group(97.50%)was higher than that in the tracheal intubation group(75.00%),and the difference was statistically significant(χ^(2)=8.538,P<0.05).After treatment,the diastolic blood pressure was(77.11±6.25)mmHg and the systolic blood pressure was(122.35±9.13)mmHg in the hemodynamic indexes of the laryngeal mask group,heart rate(86.39±10.29)beats/min,mean arterial pressure(83.35±7.49)mmHg were lower than those in tracheal intubation group,and the difference was statistically significant(P<0.05).The total incidence of complications in the laryngeal mask group was 10.00%,which was significantly lower than that in the endotracheal intubation group,which was 30.00%,and the difference was statistically significant(P<0.05).Conclusion When laryngeal mask ventilation is used in gynecological surgery,the success rate of one-time placement is high,the stress response is small,the incidence of complications can be significantly reduced,and the overall prognosis of patients can be improved.
