摘要
目的 观察白芍总苷胶囊联合阿维A胶囊治疗银屑病血虚证的临床疗效及对辅助性T淋巴细胞17(Th17)、调节性T淋巴细胞(Treg)和炎症因子的影响。方法 将94例银屑病血虚证患者按照随机数字表法分为2组,对照组47例予阿维A胶囊治疗,治疗组47例在对照组治疗基础上加白芍总苷胶囊治疗。比较2组疗效;比较2组治疗前后Th17、Treg、Th17/Treg变化;比较2组治疗前后炎症因子变化;比较2组治疗前后中医证候评分变化;比较2组不良反应发生率。结果 治疗组总有效率93.62%(44/47),对照组总有效率78.72%(37/47),治疗组疗效优于对照组(P<0.05)。2组治疗后Th17、Th17/Treg均较本组治疗前降低(P<0.05),Treg较本组治疗前升高(P<0.05);治疗后治疗组Th17、Th17/Treg均低于对照组(P<0.05),Treg高于对照组(P<0.05)。2组治疗后白细胞介素6(IL-6)、IL-17水平均较本组治疗前降低(P<0.05),IL-10水平较本组治疗前升高(P<0.05);治疗后治疗组IL-6、IL-17水平均低于对照组(P<0.05),IL-10水平高于对照组(P<0.05)。2组治疗后中医证候面色苍白、头晕目眩、心悸怔忡、唇色爪甲淡白、肢体麻木、失眠多梦、皮肤干燥、小便不利、大便燥结评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组不良反应发生率比较差异均无统计学意义(P>0.05)。结论 白芍总苷胶囊联合阿维A胶囊治疗银屑病血虚证,可调节Th17/Treg平衡,减轻炎性反应,改善中医症状,增强疗效,且不良反应少。
Objective To observe the clinical efficacy of total paeoniflorin capsules combined with Avi A capsule for psoriasis of blood deficiency syndrome and how it impacts on T helper 17 cells(Th17),regulatory T cells(Treg)and inflammatory factors.Methods Totally 94 patients with psoriasis(blood deficiency syndrome)randomly assigned into the treatment group and the control group,47 cases in each group.All patients were managed by Avi A capsule,and patients in the treatment group additionally received total paeoniflorin capsules.The changes of Th17,Treg,Th17/Treg,inflammatory factors,traditional Chinese medicine(TCM)syndrome score,the adverse incidence were included as comparators;the curative effect was assessed.Results The overall effective rate was higher in the treatment group(93.62%[44/47])compared with the control group(78.72%[37/47]),(P<0.05).After treatment,Th17 and Th17/Treg in groups were decreased(P<0.05),the reduction in the treatment group was more prevalent(P<0.05);Treg in groups was increased(P<0.05),the treatment group was more common(P<0.05);interleukin-6(IL-6),IL-17 in groups were decreased(P<0.05),while IL-10 was increased(P<0.05);lower IL-6 and IL-17 levels,and higher IL-10 levels were detected in the treatment group than those in the control group(all P<0.05);TCM syndrome(pale complexion,dizziness,palpitations,pale lip color,nail pale white,limb numbness,insomnia and dreaminess,dry skin,urine adverse,dry stool knot)scores in groups were decreased(P<0.05),which were lower in the treatment group than in the control group(P<0.05).The difference was not statistically significant in adverse incidence between groups(P>0.05).Conclusion For patients with psoriasis(blood deficiency syndrome),total paeoniflorin capsules combined with Avi A capsule can regulate Th17/Treg balance,reduce inflammatory response,improve TCM symptoms,enhance curative effect,and with few adverse reactions.
