摘要 目的探讨应用自体耳廓软骨重建耳廓软骨缺损矫正耳廓瘢痕畸形的可行性及有效性。方法回顾性分析2015年3月至2020年10月于深圳市人民医院行自体耳廓软骨重建术的耳部瘢痕疙瘩病例的临床资料,术中根据耳廓软骨缺损的位置、大小及形状取自体耳廓软骨进行重建,恢复耳廓外形,术后联合糖皮质激素局部注射治疗,观察治疗效果。结果有10例耳部瘢痕疙瘩病例耳廓软骨缺损较大,耳轮缺损长度在1.0~2.5cm之间,采用自体耳廓软骨进行重建,10例患者中有9例治愈,外观均很满意,治愈率和外观满意率均为90%,有1例复发,导致外观不满意,复发率和外观不满意率均为10%。结论采用自体耳廓软骨重建耳廓软骨缺损矫正耳廓瘢痕畸形切实可行,不仅术后成活率高、整形效果好且稳定,而且术后复发率低。 Objective To investigate the feasibility and efficacy of autogenous auricular cartilage in reconstruction for correction of auricular scar deformity.Methods In this retrospective cohort study,cases of auricular scar deformities treated with autogenous auricular cartilage reconstruction at Shenzhen People’s Hospital from March 2015 to October 2020,were reviewed.Depending on the location,size and shape of the auricular cartilage defect,cartilage flaps were harvested from the auricle for primary reconstruction and deformity correction.Postoperatively,all patients received intralesional steroid injections.Treatment outcomes were evaluated at the last follow-up.Result Auricular cartilage defects were rated as large with auricular scar deformities in 10 cases,with the length of helix defects ranging from 1.0 to 2.5cm,requiring autogenous auricular cartilage reconstructions to restore the appearances of the pinna.Outcomes were excellent in 9 cases(90%),with recurrence in 1 case(10%)resulting in unsatisfactory appearance.Conclusion It is feasible to correct auricular scar deformities with large cartilage defects using autogenous auricular cartilage,with high rates of postoperative survival and satisfying and stable cosmetic outcomes,as well as low rates of recurrence.
