呼吸肌训练结合高频胸壁振荡排痰仪排痰对老年长期卧床患者坠积性肺炎的预防效果 被引量:5

摘要 目的:观察呼吸肌训练结合高频胸壁振荡排痰仪排痰预防老年长期卧床患者发生坠积性肺炎的效果。方法:选取2019年7月至2021年3月该院收治的111例老年长期卧床患者进行前瞻性研究,按照随机数字表法分为观察组(n=56)和对照组(n=55),对照组予以高频胸壁振荡排痰仪排痰,观察组予以呼吸肌训练联合高频胸壁振荡排痰仪排痰。比较两组自行排痰率、坠积性肺炎发生率、治疗前后肺活量水平和呼吸功能指标[第1秒用力呼气容积占预计值百分比(FEV1)、最大通气量占预计值百分比(MVV)]水平。结果:观察组自行排痰率高于对照组,差异有统计学意义(P<0.05);治疗后,两组排痰量、肺活量、FEV1、MVV水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组坠积性肺炎发生率为1.79%(1/56),低于对照组的14.55%(8/55),差异有统计学意义(χ^(2)=4.472,P=0.034)。结论:呼吸肌训练结合高频胸壁振荡排痰仪排痰应用于老年长期卧床患者预防坠积性肺炎可提高自行排痰率、肺活量水平和呼吸功能指标水平,以及降低坠积性肺炎发生率,优于单纯高频胸壁振荡排痰仪排痰效果。 Objective:To observe application effects of respiratory muscle training combined with high-frequency chest wall oscillation expectoration drainage device treatment.in prevention of hypostatic pneumonia in elderly long-term bedridden patients.Methods:111 elderly long-term bedridden patients admitted to this hospital from July 2019 to March 2021 were selected for the prospective study,and were divided into observation group(n=56)and control group(n=55)according to random number table method.The control group was given high-frequency chest wall oscillation expectoration drainage device treatment,while the observation group was given respiratory muscle training combined with high-frequency chest wall oscillation expectoration drainage device treatment.The spontaneous sputum excretion rate,the incidence of hypostatic pneumonia,the levels of vital capacity and the respiratory function indexes[forced expiratory volume in the first second(FEV1),the percentage of maximum ventilation in the estimated value(MVV)]level were compared between the two groups before and after the treatment.Results:The spontaneous sputum excretion rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of expectoration,vital capacity,FEV1,and MVV in the two groups were higher than those before the treatment,those of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Further,the incidence of hypostatic pneumonia in the observation group was 1.79%(1/56),which was lower than 14.55%(8/55)in the control group,and the difference was statistically significant(χ^(2)=4.472,P=0.034).Conclusions:Respiratory muscle training combined with high-frequency chest wall oscillation expectoration drainage device treatment.in prevention of hypostatic pneumonia in elderly long-term bedridden patients can improve the self-expectation rate,the vital capacity and the respiratory function index l
