摘要
目的探究叩背联合高频胸壁振荡排痰护理法在预防患者呼吸机相关性肺炎中的效果.方法本研究采用回顾分析法,共计纳入安徽省芜湖市第二人民医院急诊ICU收治的机械通气治疗患者80例,根据患者不同的排痰方法分为对照组和治疗组,对照组患者(40例)为2018年1-3月入院,患者采取常规叩背排痰方法;治疗组患者(40例)为2018年4-10月入院,患者采取叩背联合高频胸壁振荡排痰护理法,比较2组患者排痰后1、3、5、7 d每日平均排痰量、呼吸机相关性肺炎(vent-ilator associated pneumonia,VAP)发生率及患者死亡率、其他并发症发生率、患者撤机时间、住院时间及肺部感染评分(CPIS)、患者满意率.结果对照组患者排痰后1、3、5、7 d每日平均排痰量均少于实验组,差异有统计学意义(P<0.05);治疗组患者VAP发生率(7.50%)、死亡率(7.50%)及其他并发症发生率(5.00%)均分别低于对照组(27.50%、25.00%、17.50%),差异有统计学意义(P<0.05);治疗组患者撤机时间、住院时间均短于对照组,肺部感染评分优于对照组,差异有统计学意义(P<0.05);治疗组患者满意率(95.00%)高于对照组(77.50%),差异有统计学意义(P<0.05).结论叩背联合高频胸壁振荡排痰护理法在机械通气治疗VAP患者中应用效果良好,可明显减少VAP发生率,缩短患者机械通气治疗时间,值得推广应用.
Objective To investigate the effects of back percussion combined with the nursing method of high frequency chest wall oscillation sputum expectoration on the reduction of ventilator-associated pneumonia (VAP).Methods A retrospective analysis was used in the study. Eighty patients with mechanical ventilation who were admitted to the ICU of the hospital were enrolled for the study, and were divided into the control group and the experimental group in accordance with different sputum expectoration methods, each consisting of 40 patients. The patients in the control group received simple back percussion sputum expectoration, while the patients in the experimental group received back percussion combined with the nursing method of high frequency chest wall oscillation sputum expectoration.Results The sputum volume at day 1, day 3 and day 5 after sputum expectoration, the incidence of VAP and mortality rate, the incidence of other complications, the ventilator withdrawal time, the length of hospitalization, the pulmonary infection scores (PIS) and the patient satisfaction rate were compared between the patients of the 2 groups. The sputum volume at day 1, day 3 and day 5 after sputum expectoration in the patients of the control group was less than that in the patients of the experimental group ( P <0.01). The incidence of VAP (7.50%), mortality (7.50%) and other complications (5.00%) in the patients of the experimental group were all lower than those in the control group (27.50%, 25.00%, 17.50%)( P <0.05). The ventilator withdrawal time and hospitalization time of the patients in the experimental group were all shorter than those in the control group, and the pulmonary infection scores were superior to those in the control group ( P <0.01). The patient satisfaction rate of the experimental group (95.00%) was higher than that of the control group (77.50%)( P <0.05).Conclusion Back percussion combined with the nursing method of high-frequency chest wall oscillation sputum expectoration could produce good effects in the tr
