摘要
目的 评价强化综合呼吸道管理对脑卒中相关性肺炎中的治疗效果。 方法 将73例符合脑卒中相关性肺炎诊断标准的患者，随机分成试验组和对照组，对照组按照诊疗常规给予药物治疗，试验组在药物治疗基础上强化呼吸道管理。采用临床肺部感染评分（CPIS）标准，对两组患者治疗后第1、5、10天的肺部感染情况进行评估，并按照严重程度分为三级，给予0、1、2分记录。在治疗后第10天按照病情缓解或无改善分类，评价最终治疗效果。 结果 治疗第5及第10天，试验组CPIS明显低于对照组，差异有统计学意义（5.76±2.34 vs. 6.99±2.48,3.37±2.16 vs. 5.33±2.18, P ＜0.05或0.01）；对照组在治疗第5天起，CPIS明显低于第1天，而试验组第10天才出现CPIS明显降低（5.76±2.34,3.37±2.16 vs. 7.18±2.45,5.33±2.18 vs. 7.12±2.87, P ＜ 0.05）；试验组治疗后缓解例数明显多于对照组，而无改善例数明显少于对照组（36 vs. 26,2 vs. 9, P ＜ 0.05）。 结论 强化呼吸道管理能明显改善卒中相关性肺炎患者治疗效果。
Objective： To evaluate the effects on stroke associated pneumonia undergoing intensive respiratory tract management. Methods 73 patients,according with the diagnostic criteria for stroke related pneumonia,were randomized into two groups,experimental and control group.the routine treatment with drug were applianced in the control group,and additional treatment on intensive respiratory management were used in the experimental group.An assessment,three grades with 0,1 and 2 according to lung inflammation by using clinical pulmonary infection score （CPIS）,was completed on 1st,5th and 10th respectively.Furthermore,treatment effectiveness with relief or no improvement was assessed on 10th day. Results ： CPIS in experimental group were lower than that in control group on 5th and 10th day （5.76±2.34 vs. 6.99±2.48,3.37±2.16 vs. 5.33±2.18, P 〈0.05 or 0.01）.CPIS on 5th and 10th day decreased than that on 1st day in experimental group,but only on 10th day in control group（5.76±2.34,3.37±2.16 vs. 7.18±2.45,5.33±2.18 vs. 7.12±2.87, P 〈0.05）.The treatment effectiveness with relief in experimental group was higher and the cases without any improved was lower than that in control group（36 vs. 26,2 vs. 9, P 〈0.05）. Conclusion： Intensive respiratory tract management can ameliorate stroke associated pneumonia.
