摘要
目的:探讨唇腭裂患儿家长亲职压力现状及影响因素。方法:选取2019年11月—2020年6月在国内2所三级甲等口腔专科医院就诊的144例唇腭裂患儿的家长作为研究对象,采用一般资料调查表、简式亲职压力量表、家庭环境量表进行问卷调查及亲职压力影响因素分析。结果:唇腭裂患儿家长亲职压力量表得分为(90.31±17.14)分,家庭环境量表总分为(49.24±8.66)分;相关性分析结果显示,唇腭裂患儿家长亲职压力总分、家庭环境总分及亲密度、情感表达、独立性、娱乐性、组织性呈负相关(P<0.05),与矛盾性呈正相关(P<0.05);分层回归分析结果显示,有遗传史、患儿年龄是唇腭裂患儿家长亲职压力的影响因素(P<0.05);控制人口统计学资料后,矛盾性、娱乐性为唇腭裂患儿家长亲职压力的影响因素(P<0.05),可联合解释总变异的39.6%。结论:唇腭裂患儿家长亲职压力处于临界高水平状态,与家庭环境密切相关,医护人员应帮助患儿家长营造一个和谐、融洽的家庭环境,缓解亲职压力。
Objective:To explore the status quo and influencing factors of parental stress among parents of children with cleft lip and palate(CLP).Methods:From November 2019 to June 2020,a total of 144 parents of CLP children who were admitted to two tertiary grade A stomatology hospitals in China were selected as the research subjects.A general information questionnaire,Parenting Stress Index⁃Short Form(PSI⁃SF),and Family Environment Scale⁃Chinese Version(FES⁃CV)were used to analyze the influencing factors of parental stress.Results:The PSI⁃SF score of parents of CLP children was(90.31±17.14)points,and the total score of FES⁃CV was(49.24±8.66)points.The correlation analysis showed that the total score of parenting pressure was negatively correlated with the total score of FES⁃CV intimacy,emotional expression,independence,entertainment,and organization(P<0.05),and was positively correlated with contradiction(P<0.05).The hierarchical regression analysis showed that genetic history and age of children were the influencing factors of parental stress of parents of CLP children.After controlling the demographic data,contradiction and entertainment were the influencing factors of parenting pressure of parents of CLP children(P<0.05),explaining 39.6%of the total variance.Conclusions:The parenting pressure of parents of CLP children was at a critically high level,which was closely related to the family environment.Medical staff should help parents of CLP children to create a harmonious family environment to relieve parenting pressure.
