摘要
目的探讨复方α-酮酸对维持性血液透析治疗慢性肾衰竭患者临床疗效、微炎症状态及钙磷代谢的影响。方法回顾性分析2019年5月-2021年5月行维持性血液透析治疗慢性肾衰竭151例的临床资料,根据治疗方式的不同分为研究组78例及对照组73例,对照组行维持性血液透析治疗,研究组在对照组的基础上联合复方α-酮酸治疗。比较2组临床疗效、微炎症状态[超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、钙磷代谢指标[全段甲状旁腺激素(iPTH)、血钙、血磷]以及不良反应。结果研究组临床总有效率显著高于对照组(P<0.05)。治疗后,2组hs-CRP、IL-6、TNF-α水平均较治疗前下降,以研究组下降程度最为显著(P<0.05)。治疗后,2组iPTH、血磷水平较治疗前下降,血钙水平较治疗前升高,以研究组下降或升高程度最为显著(P<0.05)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论复方α-酮酸可改善行维持性血液透析治疗慢性肾衰竭患者的钙磷代谢紊乱、微炎症状态,效果显著,且安全性较高。
Objective To investigate the effects of compoundα-Ketoicacid on clinical efficacy,microinflammatory status and calcium and phosphorus metabolism in patients with chronic renal failure(CRF)undergoing maintenance hemodialysis.Methods The clinical data of 151 cases of CRF treated with maintenance hemodialysis from May 2019 to May 2021 were retrospectively analyzed.According to different treatment methods,they were divided into the research group(n=78)and the control group(n=73).The control group received maintenance hemodialysis,while the research group was combined with Compoundα-Ketoacid treatment on the basis of the control group.The clinical efficacy,microinflammatory state[high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],calcium and phosphorus metabolism indexes[intact parathyroid hormone(iPTH),blood calcium,blood phosphorus]and adverse reactions were compared between the two groups.Results The total effective rate of the research group was significantly higher than that of the control group(P<0.05).After treatment,the levels of hs-CRP,IL-6 and TNF-αin both groups were decreased compared with those before treatment,and the decrease was the most significant in the research group(P<0.05).After treatment,the levels of iPTH and blood phosphorus in the two groups were decreased compared with those before treatment,and the level of blood calcium was increased compared with those before treatment;the decrease or increase was more significant in the research group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Compoundα-Ketoacid can improve calcium and phosphorus metabolism disorder and microinflammatory state in patients with CRF treated with maintenance hemodialysis,with significant effect and good safety.
