摘要
目的探讨Roy适应模式联合低磷饮食干预对血液透析患者心理状态、营养状况及疾病管理能力的影响。方法选取医院2020年4月—2021年4月收治的100例血液透析患者为研究对象,按照组间基本资料具有可比性的原则分为对照组和观察组,每组50例。对照组采用常规护理干预,观察组在对照组基础上实施Roy适应模式联合低磷饮食干预。干预前后采用焦虑自评量表(SAS)、抑郁自评量表(SDS)、血液透析患者自我管理量表评估患者心理状态和疾病管理能力,通过实验室检查检测患者营养指标。结果干预4周后,观察组患者焦虑、抑郁评分均低于对照组,组间比较差异有统计学意义(P<0.05)。干预4周后,观察组患者疾病管理能力各维度评分均高于对照组,组间比较差异有统计学意义(P<0.05)。干预4周后,观察组患者血清蛋白水平、前白蛋白水平和BMI指数均高于对照组,血磷水平低于对照组,组间比较差异有统计学意义(P<0.05)。结论对血液透析患者实施Roy适应模式联合低磷饮食干预,可有效减轻患者不良情绪,改善患者营养状况,提高患者疾病管理能力。
Objective To explore the effects of Roy adaptation model combined with low-phosphorus diet intervention on the psychological state,nutritional status and disease management ability of hemodialysis patients.Methods A total of 100 hemodialysis patients admitted to the hospital from April 2020 to April 2021were selected as the research objects,and divided into a control group and an observation group according to the principle of comparability of basic data between groups,with 50 cases in each group.The control group was given routine nursing intervention,while the observation group was given the Roy adaptation model combined with low-phosphorus diet intervention on the basis of the control group.Before and after the intervention,selfrating anxiety scale(SAS),self-rating depression scale(SDS),and hemodialysis patient self-management scale were used to evaluate the psychological state and disease management ability of the patients,and the nutritional indicators of the patients were detected by laboratory tests.Results After 4 weeks of intervention,the scores of anxiety and depression of the observation group were lower than those of the control group,and the dif ferences between the groups were statistically significant(P<0.05).After 4 weeks of intervention,the each dimension scores of disease management ability in the observation group were higher than those in the control group,and the dif ferences between the groups were statistically signifi cant(P<0.05).After 4 weeks of intervention,the serum protein level,prealbumin level and BMI index of the observation group were higher than those of the control group,and the serum phosphorus level was lower than that of the control group,and the dif ferences between the groups were statistically signifi cant(P<0.05).Conclusion The application of Roy adaptation model combined with low-phosphorus diet intervention in hemodialysis patients can ef fectively alleviate the patients’negative emotions,improve their nutritional status,and improve their disease management ability.
