摘要
目的分析促宫颈成熟球囊引产疗效及对分娩和新生儿结局的影响,为妇幼保健工作的开展提供参考。方法选择山东省淄博市临淄区妇幼保健院2019年4月-2020年4月引产患者,从中选取符合研究条件的82例患者作为研究对象。按数字表法随机分为对照组和观察组,每组41例,对照组的患者给予缩宫素静脉滴注治疗,观察组给予促宫颈成熟球囊引产。比较两组用药至胎儿完全娩出时间、宫颈Bishop评分、总产程时间以及产后出血量、宫颈成熟率、分娩形式以及新生儿结局情况。结果观察组用药至胎儿完全娩出时间短于对照组,宫颈Bishop评分高于对照组,差异有统计学意义(P<0.05);观察组宫颈成熟有效率以及阴道分娩率均明显高于对照组,差异有统计学意义(P<0.05);观察组新生儿不良结局发生率明显低于对照组,差异有统计学意义(P<0.05);总产程时间以及产后出血量进行比较,观察组产妇的总产程时间与对照组接近(P>0.05),观察组产妇的产后出血量明显少于对照组(P<0.05)。结论促宫颈成熟球囊引产疗效确切,可缩短引产时间,且可提高阴道分娩率和促进宫颈成熟,减少产后出血风险,对新生儿结局无不良影响,可降低新生儿窒息风险,值得推广。
Objective To analyze effect of promoting cervical maturation balloon induced labor and its influence on delivery and neonatal outcome,to provide reference for maternal and child health care.Methods The paper chose 82 patients met study conditions with induced labor in Zibo City Linzi District Maternal and Child Health Hospital from April 2019 to April 2020.Control group was treated with oxytocin intravenous drip therapy,observation group with balloon induction of labor to promote cervical maturation.Time from medication to fetus delivery,cervical Bishop scores,total duration of labor,postpartum hemorrhage amount,cervical maturity rate,delivery form and neonatal outcome were compared between two groups.Results Time from medication to fetus delivery in observation group was shorter than control group,cervical Bishop scores were higher than control group,difference was statistically significant(P<0.05).Effective rate of cervical maturation and vaginal delivery in observation group was significantly higher than control group,difference was statistically significant,(P<0.05).Incidence of adverse outcomes in observation group was significantly lower than control group,difference was statistically significant,(P<0.05).Total duration of labor in observation group was similar to control group,difference was statistically significant,(P>0.05),postpartum hemorrhage amount in observation group was significantly less than control group(P<0.05).Conclusion Promoting cervical ripening balloon induced labor can achieve accurate effect,can shorten time of induced labor,increase vaginal delivery rate,promote cervical ripening,reduce risk of postpartum hemorrhage,with no adverse effect on neonatal outcomes and less risk of neonatal asphyxia,which is worthy of promotion.
