摘要
目的探讨宫颈扩张双球囊和Foley60 mL球囊用于足月妊娠促宫颈成熟的临床疗效。方法选取2018年1月~2019年10月在我院产科收治的孕足月具有引产指征的宫颈Bishop评分<7分的单胎足月孕妇。随机分为宫颈扩张双球囊组(n=80)及Foley 60 mL球囊组(n=68)。比较两组孕妇的促宫颈成熟有效率、12 h内临产率、阴道分娩率以及宫颈评分<3分及4~6分时的阴道分娩率。结果宫颈扩张双球囊组的促宫颈成熟有效率(97.5%)高于Foley 60 mL球囊组(76.5%),差异有统计学意义(P<0.01)。宫颈扩张双球囊组的12 h内临产率(23.8%)高于Foley60 mL球囊组(7.4%),差异有统计学意义(P<0.01)。宫颈扩张双球囊组的阴道分娩率(86.0%)高于Foley60 mL球囊组(67.6%),差异有统计学意义(P<0.01)。宫颈评分≤3分时,两组经阴道分娩率比较,差异无统计学意义(P>0.05)。宫颈评分4~6分时,宫颈扩张双球囊组经阴道分娩率为92.5%,Foley60 mL组为69.5%,差异有统计学意义(P<0.01)。结论宫颈扩张双球囊对足月妊娠孕妇促宫颈成熟效果显著,尤其是宫颈评分>3分的孕妇,能有效提高阴道分娩率,降低剖宫产率。
Objective To investigate the clinical efficacy of cervical dilatation with double balloons and Foley 60 mLballoon in promoting cervical ripening in term pregnancy.Methods From January 2018 to October 2019,singleton full-term pregnant women with cervical Bishop score less than 7 points and indications of induced labor admitted to the obstetrics department of our hospital were selected.They were randomly divided into the cervical dilatation with double balloons group(n=80)and the Foley 60 mL balloon group(n=68).The effective rate of cervical ripening,labor rate within 12 hours,vaginal delivery rate,and vaginal delivery rate with the cervical score≤3 points and 4-6 points were compared between the two groups.Results The effective rate of cervical ripening in the cervical dilatation with balloons group(97.5%)was higher than that in Foley 60 mL balloon group(76.5%),with significant difference(P<0.01).The delivery rate within 12 hours in the cervical dilatation with double balloons group was 23.8%higher than that in Foley 60 mL balloon group(7.4%),with significant difference(P<0.01).The vaginal delivery rate of cervical dilatation with double balloons group(86.0%)was higher than Foley 60 mL balloon group(67.6%),with significant difference(P<0.01).When the cervical score was≤3 points,there was no significant difference in vaginal delivery rate between the two groups(P>0.05).When the cervical score was 4-6 points,the vaginal delivery rate was 92.5%in the cervical dilatation with double balloons group and 69.5%in the Foley 60 mL group,with significant difference(P<0.01).Conclusion The cervical dilatation with double balloons have a remarkable efficacy on promoting cervical maturation for full-term pregnant women,especially for pregnant women with the cervical score greater than 3 points,which effectively improves the vaginal delivery rate and reduces the cesarean section rate.
