摘要
目的 探讨血清单核细胞趋化蛋白-1、白细胞介素-6与脓毒性脑病患者脑损伤严重程度的关系。方法 将75例脓毒性脑病患者按照脑损伤程度分为轻度组、中度组、重度组,各25例,比较3组血清单核细胞趋化蛋白-1、白细胞介素-6水平,分析血清单核细胞趋化蛋白-1、白细胞介素-6水平与脓毒性脑病患者脑损伤程度的关系。结果 3组血清单核细胞趋化蛋白-1及白细胞介素-6水平比较差异均有统计学意义(P<0.01),中度组、轻度组血清单核细胞趋化蛋白-1及白细胞介素-6水平显著低于重度组(P<0.05)。相关性分析结果显示,血清单核细胞趋化蛋白-1、白细胞介素-6水平与脓毒性脑病患者脑损伤程度呈显著正相关(P<0.01);血清单核细胞趋化蛋白-1、白细胞介素-6水平单独与联合检测评估脓毒性脑病患者脑损伤程度的曲线下面积分别为0.777、0.775、0.881,均有一定评估价值,且当血清单核细胞趋化蛋白-1、白细胞介素-6的Cut-off值分别取53.335 pg·ml^(-1)、62.300 pg·ml^(-1)时,可获得最佳评估价值。结论 单核细胞趋化蛋白-1、白细胞介素-6水平与脓毒性脑病患者脑损伤程度呈显著正相关,可为临床诊断患者脑损伤程度提供一定依据。
Objective To investigate the relationship between serum monocyte chemoattractant protein-1(MCP-1),interleukin-6(IL-6)and the degree of brain injury in patients with sepsis associated encephalopathy(SAE).Methods A total of 75 SAE patients were divided into mild group,moderate group and severe group according to the degree of brain injury,with 25 cases in each group.The levels of serum MCP-1 and IL-6 in the three groups were compared,and the relationship between serum MCP-1,IL-6 levels and the degree of brain injury in SAE patients was analyzed.Results There were significant differences in the levels of serum MCP-1 and IL-6 among the three groups(P<0.01).The levels of serum MCP-1 and IL-6 in the moderate group and mild group were significantly lower than those in the severe group(P<0.05).The results of correlation analysis showed that serum MCP-1,IL-6 levels were positively correlated with the degree of brain injury in SAE patients(P<0.01).The area under curve(AUC)of serum MCP-1 and IL-6 levels alone and combined detection to evaluate the degree of brain injury in SAE patients were 0.777,0.775,and 0.881,respectively,all of which had certain evaluation values.The best evaluation value could be obtained when the cut-off values of serum MCP-1 and IL-6 were 53.335 pg·ml^(-1) and 62.300 pg·ml^(-1) respectively.Conclusions The levels of serum MCP-1 and IL-6 are significantly positively correlated with the degree of brain injury in patients with SAE,which can provide a certain basis for clinical diagnosis of the degree of brain injury in patients.
