摘要
目的 探究集束化护理干预结合感觉刺激对危重症颅脑损伤昏迷患者神经功能、脑电图的影响。方法 选择我院2020年2月至2021年3月接收的104例危重症颅脑损伤昏迷患者作为研究对象,根据护理方案将其分为对照组(常规护理联合感觉刺激)和观察组(在对照组基础上加施集束化护理干预),各52例。比较两组的护理效果。结果 干预后,两组的神经元特异性烯醇化酶(NSE)水平低于干预前,神经生长因子(NGF)及脑源性神经营养因子(BDNF)水平高于干预前,且观察组优于对照组(P<0.05)。观察组干预后脑电图Ⅰ、Ⅱ级占比高于对照组,脑电图Ⅲ、Ⅳ、Ⅴ级占比低于对照组(P<0.05)。干预后,两组的全面无反应性量表(FOUR)及格拉斯哥昏迷评分量表(GCS)评分高于干预前,且观察组高于对照组(P<0.05)。结论 集束化护理干预结合感觉刺激应用于危重症颅脑损伤昏迷患者中可有效改善神经功能及脑电图,减轻意识障碍程度。
Objective To explore the effects of cluster nursing intervention combined with sensory stimulation on neurological function and electroencephalogram of critically ill patients with craniocerebral injury and coma.Methods A total of 104 critically ill patients with craniocerebral injury and coma admitted in our hospital from February 2020 to March 2021 were selected as the research objects,and the patients were divided into control group(routine nursing combined with sensory stimulation) and observation group(cluster nursing intervention on the basis of the control group)according to the nursing plan,with 52 cases in each group.The nursing effects of the two groups were compared.Results After intervention,the level of neuron specific enolase(NSE) in the two groups was lower than that before intervention,the levels of nerve growth factor(NGF) and brain-drived neurotrophic factor(BDNF) were higher than those before intervention,and those in the observation group were better than the control group(P <0.05).After intervention,the proportion of electroencephalogram grade Ⅰ and Ⅱ in the observation group were higher than those in the control group,and the proportion of grade Ⅲ,Ⅳ and Ⅴ were lower than those in the control group(P<0.05).After intervention,the Full Outline of UnResponsiveness(FOUR) and the Glasgow Coma Scale(GCS) score of the two groups were higher than those before intervention,and those of the observation group were higher than the control group(P<0.05).Conclusion Cluster nursing intervention combined with sensory stimulation applied in critically ill patients with craniocerebral injury and coma can effectively improve the neurological function and electroencephalogram,reduce the degree of consciousness barriers.
