摘要
目的探讨银翘散联合左氧氟沙星治疗老年社区获得性肺炎的临床疗效及对免疫功能的影响。方法选取2018年7月至2021年2月我院收治的98例老年社区获得性肺炎患者为研究对象,随机分为对照组和观察组,每组49例。对照组给予左氧氟沙星片口服,观察组在对照组基础上加用银翘散水煎服。比较两组患者临床疗效、临床症状改善时间、不良反应发生率,治疗前后免疫功能水平。结果观察组患者治疗有效率(87.75%)高于对照组(73.46%)(P<0.05);观察组患者体温和白细胞数恢复正常、湿啰音消失、咳嗽消失时间与对照组相比均缩短(P<0.05);治疗后观察组和对照组患者IgA、IgG、CD4^(+)T细胞、CD4^(+)/CD8^(+)、NK均上升,CD8^(+)T细胞下降,观察组IgA、IgG、CD4^(+)T细胞、CD4^(+)/CD8^(+)、NK水平高于对照组,CD8^(+)T细胞低于对照组(P<0.05);对照组患者总不良反应发生率(18.36%)高于观察组(4.08%)(P<0.05)。结论银翘散联合左氧氟沙星治疗老年社区获得性肺炎,能够提高治疗有效率,缩短临床症状改善时间,降低不良反应发生率,改善机体免疫功能。
Objective To explore the clinical efficacy of Yinqiaosan combined with levofloxacin in the treatment of elderly community-acquired pneumonia and its influence on immune function.Methods Ninety-eighty elderly patients with community-acquired pneumonia admitted to our hospital from July 2018 to February 2021 were selected as the research objects and were divided into the control group and the observation group according to the random number table method,with 49 cases in each group.The control group was given levofloxacin tablets orally,and the observation group was given decocted Yinqiaosan besides levofloxacin tablets.The clinical efficacy,the time to improve clinical symptoms,the incidence of adverse reactions,and the level of immune function before and after the treatment were compared between the two groups.Results The effective rate of the treatment in the observation group(87.75%)was higher than that of the control group(73.46%)(P<0.05);the time for body temperature to return to normal,for white blood cell count to return to normal,and for the disappearance of wet rales and cough in the observation group was shorter than that in the control group(P<0.05);after the treatment,IgA,IgG,CD4^(+),CD4^(+)/CD8^(+),and NK increased,while CD8^(+)decreased in both groups;the levels of IgA,IgG,CD4^(+),CD4^(+)/CD8^(+),and NK in the observation group were higher than those in the control group,while CD8^(+)was lower than that in the control group(P<0.05);the total adverse reaction rate in the control group(18.36%)was significantly higher than that in the observation group(4.08%)(P<0.05).Conclusion Yinqiaosan combined with levofloxacin in the treatment of elderly patients with community-acquired pneumonia can improve the effective rate of the treatment,shorten the time of clinical symptom improvement,reduce the incidence of adverse reactions,and improve the immune function of the body.
