慢性牙周炎合并牙龈卟啉单胞菌感染HSP60及TLR4信号通路的表达及意义 被引量:4

Expression of HSP60 and TLR4 signaling pathway-related cytokines in chronic periodontitis patients complicated with Porphyromonas gingivalis infection

摘要 目的 研究慢性牙周炎(CP)合并牙龈卟啉单胞菌(P.gingivalis)感染患者热休克蛋白60(HSP60)及Toll样受体4(TLR4)信号通路相关细胞因子表达水平及其临床意义。方法 选取2018年9月-2020年9月医院口腔科收治的108例CP患者为样本开展横断面研究,同时选取体检健康人群60名为对照组,检测入组研究对象牙龈卟啉单胞菌阳性率以及牙周组织TLR4、髓样分化因子88(MyD88)、HSP60、白细胞介素1β(IL-1β)和IL-6水平。结果 CP组牙龈卟啉单胞菌阳性率高于对照组,且随着病情严重程度增加而升高,差异有统计学意义(P<0.05);牙龈卟啉单胞菌感染CP患者牙周组织TLR4阳性率以及MyD88、HSP60、IL-1β和IL-6水平高于未感染患者(P<0.05);牙龈卟啉单胞菌感染CP患者牙周组织TLR4和MyD88表达水平与HSP60、IL-1β和IL-6水平具有正相关性(P<0.05);CP患者牙周组织HSP60、IL-1β和IL-6相对表达量诊断牙龈卟啉单胞菌感染的曲线下面积(AUC)分别为0.739、0.639和0.696,三者联合检测为0.806。结论 CP合并牙龈卟啉单胞菌感染患者HSP60表达水平升高且TLR4/MyD88信号通路明显激活,检测HSP60、IL-1β和IL-6水平对判断牙龈卟啉单胞菌感染具有良好参考价值。 OBJECTIVE To observe the expression levels of heat shock protein 60(HSP60) and Toll-like receptor 4(TLR4) signaling pathway-related cytokines in chronic periodontitis(CP) patients complicated with Porphyromonas gingivalis(P. gingivalis) infection and analyze the clinical significance. METHODS A total of 108 CP patients who were treated in stomatology department from Sep 2018 to Sep 2020 were enrolled in the study, and a cross sectional survey was conducted for the patients, meanwhile, 60 healthy people who received physical examination were chosen as the control group. The positive rate of P. gingivalis and the expression levels of TLR4, myeloid differentiation factor 88(MyD88), HSP60, interleukin-1β(IL-1β) and IL-6 in periodontal tissues were detected for the enrolled study objects. RESULTS The positive rate of P. gingivalis of the CP group was significantly higher than that of the control group and was increased with the severity of disease, difference(P<0.05). The positive rate of periodontal tissue TLR4 and expression levels of MyD88, HSP60, IL-1β and IL-6 were significantly higher in the CP patients with P. gingivalis infection than in the CP patients without P. gingivalis infection(P<0.05). The expression levels of periodontal tissue TLR4 and MyD88 of the CP patients with P. gingivalis infection were positively correlated with the levels of HSP60, IL-1β and IL-6(P<0.05).The areas under curves(AUCs) of the periodontal tissue HSP60, IL-1β and IL-6 relative expression levels were respectively 0.739, 0.639 and 0.696 in diagnosis of P. gingivalis infection among the CP patients, with the joint detection of the three indexes 0.806. CONCLUSION The expression level of HSP60 of the CP patients complicated with P. gingivalis infection is elevated, and the TLR4/MyD88 signaling pathway is significantly activated. The detection of expression levels of HSP60, IL-1β and IL-6 may facilitate the diagnosis of P.gingivalis infection.
