鼻息肉鼻窦炎患者金黄色葡萄球菌感染率及其毒力和耐药基因 被引量:1

Incidence rate of Staphylococcus aureus infection in chronic rhinosinusitis and nasal polyps patients, virulence and drug resistance genes

摘要 目的 分析鼻息肉鼻窦炎(CRSwNP)患者金黄色葡萄球菌的毒力和耐药基因。方法 以淄博市第一医院2018年6月-2020年6月收治的93例CRSwNP患者为研究对象,分析金黄色葡萄球菌检出率及其药敏结果,并对耐药基因及毒力基因进行分析。结果 93例CRSwNP患者检出金黄色葡萄球菌42株,感染率为45.16%,其中耐甲氧西林金黄色葡萄球菌(MRSA)检出14株,检出率为33.33%。CRSwNP患者检出的金黄色葡萄球菌对青霉素、红霉素、克林霉素的耐药性较高。金黄色葡萄球菌检出mecA、tetM、ermA、ermC耐药基因型分别为33.33%、16.67%、2.38%、11.90%;毒力基因sea、sec、seb、sed、hlg、pvl、tst检出率分别为38.10%、28.57%、83.33%、28.57%、19.05%、7.14%、23.81%。结论 CRSwNP患者金黄色葡萄球菌感染率相对较高,金黄色葡萄球菌对常见抗菌药物耐药率较高,其耐药性主要与耐甲氧西林、四环素类、耐大环内酯类耐药基因关系密切,且肠毒素基因检出率较高,值得临床重视。 OBJECTIVE To analyze the virulence and drug resistance genes in Staphylococcus aureus strains isolated from patients with chronic rhinosinusitis and nasal polyps(CRSwNP). METHODS A total of 93 patients with CRSwNP who were treated in Zibo First Hospital from Jun 2018 to Jun 2020 were recruited as the study subjects, the isolation rates of S.aureus and result of drug susceptibility testing were observed, and the drug resistance genes and virulence genes were analyzed. RESULTS Totally 42 strains of S.aureus were isolated from 93 patients with CRSwNP, the infection rate was 45.16%, and 14 strains of methicillin-resistant Staphylococcus aureus(MRSA) were isolated, with the isolation rate 33.33%. The S.aureus strains isolated from the CRSwNP patients were highly resistant to penicillin, erythromycin and clindamycin. The detection rates of drug resistance genotypes mecA, tetM,ermA and ermC were 33.33%, 16.67%, 2.38% and 11.90%, respectively;the detection rates of virulence genes sea, sec, seb, sed, hlg, pvl and tst were 38.10%,28.57%, 83.33%, 28.57%, 19.05%, 7.14% and 23.81%, respectively. CONCLUSION The incidence rate of S.aureus infection is relatively high among the CRSwNP patients, the drug resistance rates of S.aureus strains to the commonly used antibiotics are relatively high, and the drug resistance is closely associated with the methicillin-resistant, tetracyclines and macrolides-resistant genes, the detection rate of enterotoxin genes is high.
